Inter-relações entre tabagismo, sintomas depressivos e genética

Abstract
INTRODUÇÃO: O tabagismo é a maior causa de morte e de doenças preveníveis. A grande variabilidade fenotípica do tabagista deve-se à diversidade dos estímulos ambientais, hábitos e condições pessoais, como eventuais doenças psiquiátricas e aos polimorfismos genéticos. O tabagismo está relacionado com depressão, mas a direção desta relação não está clara. Para a identificação de loci que possam aumentar o risco para o tabagismo, muitos estudos de associação genômica ampla vêm sendo realizados para acessar as variáveis relacionadas ao tabagismo. Numerosos SNPs têm sido identificados, mas somente poucos destes foram replicados em estudos independentes. OBJETIVO: Verificar se há associação entre tabagismo e sintomas depressivos, bem como replicar os polimorfismos genéticos relacionados com o tabagismo, previamente descritos, em uma população brasileira. MÉTODOS: Dois estudos, transversal e caso-controle, foram realizado no Hospital São Lucas da PUCRS, Porto Alegre, em duas amostras constituídas de 1021 e 531 indivíduos, não relacionados, doadores de sangue. Os voluntários completaram questionário padrão com variáveis demográficas, relacionadas ao tabagismo e a sintomas depressivos (somente no primeiro estudo). Vinte e um SNPs (Single Nucleotide Polymorphism) escolhidos foram genotipados usando a plataforma Sequenom MassARRAY iPLEX e os dados analisados pelos programas SPSS e Plink na University of Toronto, Canadá. O nível de significância foi de 0.05. RESULTADOS: Verificou-se associação entre tabagistas atuais e sintomas depressivos e que ex-fumantes têm BDI (Beck Depression Inventory) menor que fumantes atuais (P<0.001). Todos os SNPs, tanto em casos como em controles, estavam em equilíbrio de Hardy Weinberg. Diferenças significativas entre fumantes e não fumantes foram detectadas nos SNPs rs10836358 (P=0.047) no gene SLC1A2 e rs2268983 (P=0.033) no gene ACTN1. CONCLUSÃO: Ex-fumantes têm BDI menor que fumantes atuais. Os SNPs estudados fornecem a primeira confirmação da associação dos polimorfismos rs10836358 e rs2268983 e tabagismo, sugerindo que estes dois genes podem ser relevantes clínica e fisiologicamente
