Abstract
Introdução: A dor pélvica crônica (DPC) é definida como dor recorrente ou contínua na região inferior do abdome ou pelve, não menstrual ou não cíclica, com duração de pelo menos seis meses. Há fortes evidências de que até 85% das pacientes com DPC apresentam sérias disfunções do sistema músculo-esquelético, dentre elas a síndrome miofascial abdominal (SMFA). A SMFA é caracterizada como dor abdominal intensa e profunda, originada por pontos gatilho hiperirritáveis, localizados, geralmente, dentro de uma faixa musculoesquelética ou sua fáscia de revestimento. Na literatura, existem poucos estudos que abordam a SMFA. Objetivos: Avaliar e comparar a eficácia do ultrassom terapêutico e injeção de anestésico local na melhora da dor em mulheres com síndrome miofascial abdominal secundária à dor pélvica crônica. Materiais e métodos: Foi conduzido um ensaio clínico randomizado, onde as pacientes são alocadas para dois tipos de tratamento, Grupo A: ultrassom terapêutico (US) e Grupo B: injeção de anestésico local (IA). Os instrumentos que foram utilizados para avaliação e reavaliação são a Escala Visual Analógica de dor (E.V.A), Escala Categórica Numérica de dor (E.C.N), Escala McGill de dor e o questionário de avaliação de qualidade de vida SF-36. Resultados: Na E.V.A o grupo US iniciou o tratamento com média de dor 7,22 e finalizou o tratamento com média de 4,33, o grupo IA iniciou o tratamento com média de dor 6,70 e finalizou o tratamento com média de 3,68. O ultrassom terapêutico e a injeção de anestésico local foram eficazes na redução da dor clínica e na melhora da qualidade de vida por meio das variáveis analisadas entre as participantes do estudo. Não houve diferença entre os grupos. Conclusão: O tratamento com ultrassom terapêutico e injeção de anestésico local foram eficazes na redução da dor clínica e melhora da qualidade de vida em mulheres com SMFA secundária à DPC.
