A perda gestacional recorrente (PGR) é uma condição multifatorial que afeta até 5% dos casais. Evidências sugerem que o estresse oxidativo (EO) pode contribuir para distúrbios reprodutivos. Uma revisão sistemática e metanálise (RSMA) foi conduzida para sintetizar as evidências atuais sobre a relação entre EO seminal e PGR. Objetivo: Avaliar se os marcadores seminais de EO diferem entre homens de casais com PGR e aqueles de controles férteis. Metodologia: Esta revisão sistemática aderiu às diretrizes do PRISMA. Bancos de dados, incluindo PubMed, EMBASE e Web of Science, foram pesquisados em busca de estudos que examinassem a EO seminal em homens cujos parceiros experimentaram PGR. A Escala de Newcastle-Ottawa (NOS) foi utilizada para avaliar a qualidade dos estudos incluídos. Uma metanálise foi realizada usando o Review Manager (RevMan) para calcular estimativas agrupadas da diferença média padronizada (DMP) para vários marcadores EO, como espécies reativas de oxigênio (EROs) e capacidade antioxidante total (CAT). As análises de subgrupos foram realizadas com base nos ensaios usados para medir a EO. Resultados: Um total de sete estudos foram incluídos, compreendendo 243 homens do grupo PGR e 216 controles. A metanálise mostrou que os níveis de marcadores pró-oxidantes foram significativamente elevados em homens do grupo PGR em comparação com os controles (DMP = 1,59, IC 95% = 1,30 a 1,89, p < 0,00001, efeitos fixos; DMP = 1,80, IC 95% = 0,04 a 3,55, p = 0,04, efeitos aleatórios). Os marcadores antioxidantes foram significativamente reduzidos no grupo PGR (DMP = -1,04, IC 95% = -1,32 a -0,75, p < 0,00001, efeitos fixos; DMP = -1,46, IC 95% = -2,39 a -0,52, p = 0,002, efeitos aleatórios). A heterogeneidade entre os estudos foi alta. O viés de publicação foi detectado com base no gráfico de funil.
