ABSTRAK Latar belakang : Penyakit neurodegeneratif merupakan kumpulan kondisi progresif yang merusak atau membunuh sel saraf di otak dan sumsum tulang belakang, yang kerusakannya menyebabkan hilangnya fungsi secara bertahap dan ireversibel. Kematian sel saraf pada penyakit neurodegenaratif ini disebabkan oleh banyak faktor, diantaranya bertambahnya usia, genetik, reactive oxcigen species ( ROS), synaptik injury, kerusakan mitokondria di level seluler, protein misfolding dan aggregation, inflamasi kronis. Tujuan: Penelitian ini adalah untuk menganalisis pemberian katekin gambir (uncaria gambir roxb) terhadap fungsi kognitif, ekspresi gen APP, MAPT, BDNF, IL-10, TGF- β dan kadar MDA pada lanjut usia (lansia). Metode : Penelitian ini menggunakan desain Double-blind Randomized Conrtol Trial (RCT) untuk menguji efek pemberian 224 mg/hari katekin gambir selama 12 minggu asupan harian. Studi ini melibatkan lansia sebanyak 58 orang yang berumur ≥ 60 tahun di Padang, dengan skor Montreal Cognitive Assesment (MoCA) versi Indonesia dengan kriteria inklusi skor MoCA-Ina ≥ 20 ≤ 25. Subjek dibagi dalam dua kelompok, 29 orang subjek kelompok intervensi yang diberi katekin gambir dan 29 orang subjek pada kelompok kontrol yang diberi plasebo. Pengukuran ekspresi gen APP, MAPT,BDNF, IL-10, TGF- β diukur menggunakan RT-PCR dan kadar MDA plasma diukur menggunakan spektrofotometer sebelum dan sesudah pemberian katekin gambir. Hasil : Penelitian ini menunjukkan hasil terjadi peningkatan fungsi kognitif pada kelompok katekin dengan nilai signifikansi p< 0,001. Terdapat pengaruh pemberian katekin gambir terhadap ekspresi gen APP, IL-10, TGF- β dan kadar MDA plasma dengan nilai signifikansi p=0,011, p< 0,001, p< 0,001 dan p<0,001 dan tidak terdapat pengaruh pemberian katekin gambir terhadap ekspresi gen MAPT dan BDNF dengan nilai signifikansi p= 0,104 dan p= 0,135. Kesimpulan : Terdapat pengaruh pemberian katekin gambir terhadap peningkatan fungsi kognitif, terdapat pengaruh pemberian katekin gambir terhadap penurunan ekspresi gen APP dan kadar MDA plasma, terdapat pengaruh pemberian katekin gambir terhadap peningkatan ekspresi gen IL-10, TGF-β, namun tidak terdapat pengaruh pemberian katekin gambir terhadap ekspresi gen MAPT dan BDNF pada lansia.
