RésuméEn 1983 paraît une étude américaine montrant que si les problèmes de roulement élevé du personnel infirmier étaient déjà répandus, certains hôpitaux s’en tiraient nettement mieux. Ils recrutaient et gardaient assez facilement, de là leur nom d’hôpitaux magnétiques.Pourquoi eux ? Encore aujourd’hui, la littérature évaluative est d’une grande cohérence quant aux variables critiques à la base de leur succès. Celles constituant des incitatifs non-économiques (comme la valorisation et le soutien) sont d’aussi puissants, sinon de plus puissants, « prédicteurs » de comportements que les variables jouant sur le revenu. Ces hôpitaux paient bien, mais rien de hors-normes. Ce qui est exceptionnel, c’est le milieu de travail et la satisfaction à y évoluer.Dès l’étude initiale, les auteurs notent la convergence entre les réponses des infirmières et celles de la direction, sur les éléments qui font que des hôpitaux sont magnétiques. La lecture de la réalité est similaire. Dès lors, a priori, on fait assez confiance et ne cherche pas un éventuel agenda caché. C’est une gestion par les « gestes et l’exemple ».Les traits de « bon employeur » des hôpitaux magnétiques permettent de soutenir que la réalité décrite s’applique à l’ensemble du personnel. Le personnel infirmier devient un cas traceur révélateur de l’ensemble. Ces portraits canadien, européen ou encore de sous-systèmes américains (comme l’exemple de Kaiser Permanente) montrent que des éléments systémiques jouent. Le tout n’est pas qu’affaire de gestion d’établissements isolés. Le dossier hôpitaux magnétiques ne doit pas servir à dédouaner les administrations centrales.
