Test Viral HPV en Dépistage Primaire du Cancer du Col de l’Utérus à Dakar (Sénégal) : A Propos de 505 Cas

Abstract
RÉSUMÉ Introduction. Le cancer du col de l’utérus est une néoformation maligne développée au dépend du col utérin. Cette étude avait pour objectif de décrire les caractéristiques socio-épidémiologiques des patientes ayant bénéficié d’un dépistage du cancer du col utérin par le test viral HPV, de préciser les aspects diagnostiques, thérapeutiques et le suivi des patientes ayant un test viral HPV positif et de décrire les facteurs associés à l’infection à HPV. Méthodologie. Il s'agissait d'une étude rétrospective, descriptive et analytique menée à la maternité du Centre de Santé Gaspard KAMARA du 07 septembre 2020 au 19 janvier 2022 sur une période de 16 mois. La saisie des données a été faite avec le logiciel Epi info version 7 et l’analyse avec le logiciel SPSS version 21. Résultats. Une étude portant sur 505 femmes (âge moyen : 43,2 ans) a montré que 73,1% étaient âgées entre 30 et 50 ans et 82% étaient mariées. L’âge moyen au premier rapport sexuel était de 23,9 ans, avec 48,1% entre 18 et 24 ans. La gestité moyenne était de 3, avec 43% de multigestes. Le test HPV était positif chez 15,3% des femmes, dominé par les types HPV 18-45 (22,1%) et HPV 16 (9,1%). Parmi les HPV positives, 88,3% ont eu un test IVA, dont 53,2% étaient positives. La colposcopie a révélé des lésions chez plusieurs patientes, et trois biopsies ont montré : CIN 1, CIN 2 et un polype bénin. Sur le plan thérapeutique, 57,1% ont eu une thermo-ablation, et une hystérectomie a été réalisée pour un cas de HSIL. Conclusion. Le cancer du col de l’utérus reste une pathologie fréquente dans le monde. Elle pose un problème de santé publique. Il s’agit d’une affection évitable d’où l’intérêt d’un dépistage primaire reposant entre autres sur le test viral HPV. ABSTRACT Introduction. This was a retrospective, descriptive, and analytical study conducted at the maternity ward of the Gaspard Kamara Health Center from September 7, 2020, to January 19, 2022, over a period of 16 months. Data entry was performed using Epi info version 7 software and analysis using SPSS version 21 software. Results. A study involving 505 women (mean age: 43.2 years) showed that 73.1% were aged between 30 and 50 years, and 82% were married. The mean age at first sexual intercourse was 23.9 years, with 48.1% between 18 and 24 years. The average number of pregnancies was 3, with 43% being multiparous. The HPV test was positive in 15.3% of women, with HPV types 18-45 (22.1%) and HPV 16 (9.1%) being the most common. Among HPV-positive women, 88.3% underwent VIA testing, of whom 53.2% tested positive. Colposcopy revealed lesions in several patients, and three biopsies showed CIN 1, CIN 2, and a benign endocervical polyp. Therapeutically, 57.1% underwent thermo-ablation, and one case of HSIL was treated with hysterectomy. Conclusion. Cervical cancer remains a common disease worldwide. It poses a public health problem. This is a preventable condition, hence the importance of primary screening based, among other things, on the HPV viral test.
