Abstract
Günümüzde yaşlanan nüfus, artan obezite oranları, kronik hastalıklara bağlı sedanter yaşam tarzı osteoartrit'in önemini artırmaktadır. Özellikle yaşlı ve kırılgan hastaların taburculuk sonrası bakım gereksinimlerinin artması nedeniyle transisyonel bakım modeli sağlık sisteminde hasta bakımının önemli bir yönü haline gelmiştir. Bu çalışma total diz artroplastisi olan hastalarda transisyonel bakım uygulamasının hastaların fonksiyonel durum, öz-etkililik algısı ve sağlık hizmeti kullanımı üzerindeki etkinliğini belirlemek amacıyla, Şubat 2021- Kasım 2021 tarihleri arasında yapılmış, klinik randomize kontrollü bir çalışmadır. Çalışmanın katılımcılarını üçüncü basamak sağlık hizmeti veren bir hastanenin Ortopedi ve Travmatolojı Cerrahisi Kliniği'ne total diz artroplasti ameliyatı olmak için yatan hastalar oluşturmuştur. Örnekleme kabul edilme kriterleri doğrultusunda diz artroplastisi olacak olan hastalar blok randomizasyon yöntemi ile kontrol (n=35) ve deney (n=35) gruplarına atanmıştır. Deney grubu transisyonel bakım alırken kontrol grubu standart bakım almıştır. Hasta çıktılarının zamana bağlı değişimleri (ameliyat öncesi, taburculuktan 2 ve 9 hafta sonra) WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index) ve AÖÖ (Artritlerde Öz-etkililik Ölçeği) ölçekleri kullanılarak değerlendirilmiş, taburculuk sonrası 2.ci, 6.cı, 9.cu hafta ve 3.cü ayda hastaların hastaneye başvurup başvurmadıkları sorgulanmıştır. Deney grubundaki katılımcılar (WOMAC¬=T2:53.60, T3:31.09; AÖÖ=T2:129.69, T3:161.00), kontrol grubuna kıyasla (WOMAC=T2:67.97, T3:42.97; AÖÖ=T2:110.29, T3:138.51) fonksiyonel durum ve öz-etkililik algısında önemli ölçüde daha fazla iyileşme göstermiştir (p<0.05). Ek olarak deney grubunun hastaneye başvuru oranlarının kontrol grubuna göre daha düşük olduğu izlenmiş, 6.cı haftadaki ölçümde istatistiksel olarak anlamlı bir fark görülmüştür (p<0.05). Transisyonel bakım uygulanan artroplasti hastalarının fonksiyonel durum ve öz-etkililik algılarında önemli bir iyileşme olduğu, aynı şekilde hastaneye başvuru oranlarının da azaldığı belirlenmiştir. Bu sonuç transisyonel bakım modelinin total diz artroplastisi olan hastalarda uygulanmasının teşvik edilmesi gerektiğini göstermektedir.
