Alzheimer hastalarının beslenme durumunun fiziksel ve bilişsel fonksiyonlarla ilişkisinin değerlendirilmesi

Abstract
Bu çalışmanın amacı Alzheimer hastalarının (AH) beslenme durumunu saptamayı, beslenme ile sarkopeni, depresyon, kırılganlık, bilişsel fonksiyonlarla ilişkili faktörlerin belirlenmesidir. Araştırma Gaziantep ili Moral Evine kayıtlı çalışmaya katılmayı kabul eden 62 (31 erkek, 31 kadın) bireyle yürütülmüştür. Soru formu ile bireylerin; sosyo-demografik ve sağlık durumuna ilişkin bilgiler, beslenme durumunun değerlendirilmesi için Mini Nütrisyonel Değerlendirme (MNA), 24 saatlik besin tüketimi, antropometrik ölçümler, psikososyal ve bilişsel durumun değerlendirilmesi için Mini Mental Değerlendirme Testi (MMDT), Beck Depresyon Ölçeği, Beck Anksiyete Ölçeği, biyofiziksel değerlendirme için Edmonton Kırılganlık Ölçeği, Sarkopeni Hızlı Tarama Testi (SARC-F) uygulanmıştır. Hastaların yaş ortalaması 75,3±6,1 bulunmuş olup, %83,9'u 70 yaş ve üzerindir. Beden kütle indekslerine göre %74.2 si normal ağırlığının üstündedir ve yetersiz beslenme tespit edilmemiştir ancak MNA'ya göre %56,5'i malnütsiyon riski altında %3,2'si malnütrisyonludur. MNA puanı ile enerji ve protein alımı arasında pozitif yönde orta düzey korelasyon varken (r=.309; p=.015), (r=.388; p=.002); Edmonton kırılganlık puanı ile negatif yönde orta düzey korelasyon bulunmuştur (r=-.361; p=.004). Beden Kütle İndeksi (BKİ) ile enerji alımı arasında negatif yönde düşük düzey korelasyon bulunmuştur (r=-.299; p=.018). Enerji alımı ile MNA puanı pozitif yönde orta (r=.309; p=.015) ; protein alımı ile pozitif yönde düşük ilişki (r=.704; p=.000) ve beden kütle indeksi ile negatif yönde düşük düzeyde (r=-.299; p=.018), SARC-F ile pozitif yönde orta düzeyde ilişki bulunmuştur (r=.323; p=.011). Protein alımı ile MNA puanı arasında pozitif yönde orta düzey ilişki (r=.388; p=.002), enerji alımı ile pozitif yönde orta ilişki bulunmaktadır (r=.704; p=.000). El kavrama gücü ile Edmonton kırılganlık puanı (r=-.307; p=.015) ve SARC-F puanı arasında negatif yönde orta düzeyde ilişki bulunmuştur (r=-.415; p=.001). Edmonton kırılganlık puanı ile MNA puanı arasında negatif yönde (r=-.361; p=.004), el kavrama gücü ile negatif yönde (r=-.307; p=.015), SARC-F ile negatif (r=-.415; p=.001), depresyon puanı ile pozitif yönde orta düzeyde ilişki bulunmuştur (r=.304; p=.016). SARC-F puanı ile enerji alımı negatif yönde düşük düzeyde (r=-.266; p=.027), el kavrama gücü negatif yönde(r=-.415; p=.001), edmonton kırılganlık puanı pozitif yönde (r=.634; p=.000), anksiyete ve depresyon puanları pozitif yönde orta düzeyde korelasyon bulunmuştur (r=.303; p=.017), (r=.263; p=.039). Anksiyete puanı ile SARC-F,depresyon mini mental durum testi ile pozitif yönde düzeyde orta düzeyde korelasyon bulunmuştur (r=.303; p=.017), (r=.620; p=.000), (r=.320; p=.011). Depresyon puanı ile Edmonton kırılganlık puanı (r=.304; p=.016) ve anksiyete (r=.620; p=.000) ile pozitif yönde orta düzeyde SARC-F ile negatif yönde düşük düzeyde korelasyon bulunmuştur (r=.263; p=.039). Mini mental değerlendirme testi, anksiyete ile pozitif yönde orta düzeyde korelasyon bulunmuştur (r=.320; p=.011). Bu çalışmada beslenme durumu ile ilişkilere bakıldığında malnütrisyon; enerji ve protein alımı, sarkopeni ve BKİ ile ilişkili bulunmuştur. Sarkopeni ise enerji, el kavrama, kırılganlık, anksiyete ve depresyon ile ilişkili bulunmuştur. Alzheimer hastalarının beslenme durumunun değerlendirilmesinde antropometrik ölçüm dışında, biyofizik testler ve tarama testlerinin yapılması önemlidir. Malnütrisyonun yanı sıra sarkopeni varlığında da besin öğesi alımının değerlendirilmesi önerilir.
