Management and treatment of ear mite infestation (Otodectes cynotis) in domestic hedgehogs: A case report

Approximately 3 to 6 percent of exotic companion mammals presented to veterinary clinics in North America are diagnosed with ectoparasitic infestations, yet species-specific treatment protocols for many of these animals remain poorly defined. This case report documents the management and treatment of an Otodectes cynotis ear mite infestation in a 16-month-old female African pygmy hedgehog (Atelerix albiventris) presented to the Vancouver Institute of Animal Sciences with a three-week history of progressive otic discharge, head tilting, and reduced appetite. Clinical examination revealed bilateral otitis externa with copious dark-brown ceruminous debris, and microscopic evaluation of cerumen samples confirmed the presence of adult O. cynotis mites, nymphal stages, and numerous ova. The hedgehog had been kept in a household alongside two domestic ferrets, one of which had recently received treatment for ear mites, suggesting a possible cross-species transmission event. Treatment was initiated using a multimodal approach consisting of subcutaneous ivermectin at 0.3 mg/kg administered on days 0, 10, and 20, paired with twice-weekly bilateral ear irrigation using a veterinary-grade ceruminolytic agent. A comprehensive clinical assessment protocol was implemented, with evaluations performed every five days throughout a 40-day observation period. Parasitological monitoring revealed an initial mean mite density of 52.3 organisms per microscopic field, which declined progressively to 33.8 at day 5, 19.6 at day 10, 8.4 at day 15, 3.1 at day 20, 0.8 at day 25, and 0.0 by day 30. Clinical severity indices for the five principal signs of otoacariasis, namely pruritus, ceruminous discharge, head shaking, canal erythema, and periauricular crusting, all demonstrated statistically significant reductions between baseline and final assessment (p<0.001 for each parameter). Body condition improved markedly, with body weight increasing from 289 g to 331 g (a 14.5% gain), and behavioral activity indices rose from 1.1 to 3.7 on a standardized four-point scale. No adverse reactions to ivermectin were observed at any point during the treatment course. Follow-up assessment at day 40 confirmed sustained parasitological clearance with no evidence of re-infestation. The findings of this research support the efficacy of a combined systemic-topical treatment strategy for O. cynotis management in domestic hedgehogs and highlight the importance of considering multi-pet household dynamics in ectoparasite transmission [9].
