Evaluation of late postoperative results on patients with aortic stentless valve

Abstract
Toronto bestentinis vožtuvas sukurtas siekiant pagerinti kraujotaką protezuojant aortos vožtuvą, maksimaliai padidinant galimą tėkmės plotą, sumažinant transvožtuvinį gradientą, kad būtų pasiekta geresnių ikioperacinių ir pooperacinių išgyvenimo rezultatų. Metodai. Ikioperacinių ir pooperacinių kraujotakos pokyčių įvertinimui 21 iš 47 pacientų, kuriems po izoliuotos ar kombinuotos (aortos-vainikinių jungčių suformavimas, mitralinio vožtuvo plastika, triburio vožtuvo plastika, miego arterijos tromboendoterektomija) aortos vožtuvo protezavimo operacijos, buvo atlikta echokardioskopija 7–10 dienų iki operacijos ir praėjus 6 bei 12 mėnesių po operacijos. Rezultatai. Vidutinis operuotų ligonių amžius – 68,5 metų. Vidutinis implantuoto Toronto bestentinio vožtuvo dydis vyrams – 26,6 mm, moterims – 24 mm. Ankstyvuoju pooperaciniu laikotarpiu mirė tik vienas pacientas, o vėlyvuoju nemirė nė vienas. Praėjus 12 mėnesių po operacijos, vidutinio transvožtuvinio aortos gradientas sumažėjo nuo 40,12 iki 8,63 mmHg ir buvo lydimas kairiojo skilvelio užpakalinės sienos galinio diastolinio dydžio sumažėjimo nuo 14,5 iki 11,6 mm, o tarpskilvelinės pertvaros galinio diastolinio dydžio – nuo 15,6 iki 3 mm. Echokardiografiškai užfiksuotas žymus kairiojo skilvelio masės sumažėjimas nuo 417,3 iki 330,7 g, kairiojo skilvelio masės indeksas pakito nuo 224,8 iki 172,6 g. Po 12 mėnesių nebuvo nei ankstyvojo, nei vėlyvojo pooperacinio aortos vožtuvo nepakankamumo, išskyrus du pacientus, kuriems nustatytas I° nepakankamumas. Išvados. Ankstyvojo ir vėlyvojo pooperacinio laikotarpio echokardiografinio tyrimo rezultatai parodė ryškią tendenciją kairiojo skilvelio masės indekso mažėjimo ir hipertrofijos. Bestentinių protezų panaudojimas, kai nereikia vartoti antikoaguliantų po operacijos, gali pagerinti vėlyvuosius pooperacinius rezultatus.
