Problemos aktualumas ir darbo tikslas. Obstrukcinė miego apnėja (OMA) – kvėpavimo sutrikimas, pasireiškiantis miego metu, kai dėl kvėpavimo takų nepilno ar visiško susiaurėjimo laikinai nutrūksta oro srautas į plaučius. Apnėjos epizodai gali bloginti gyvenimo kokybę ir didinti įvairių sveikatos sutrikimų riziką. Ortognatinė operacija yra vienas iš gydymo būdų, padedantis sušvelninti OMA simptomus, koreguojant žandikaulių padėtį ir taip atkuriant normalų kvėpavimo takų praeinamumą. Šio darbo tikslas buvo išanalizuoti ortognatinės chirurgijos efektyvumą gydant OMA, remiantis priešoperaciniais ir pooperaciniais apnėjos–hipopnėjos indekso ir Epworth dienos mieguistumo skalės rezultatais. Medžiaga ir metodai. Mokslinės literatūros paieška ir apžvalga atlikta laikantis PRISMA gairėmis trijose elektroninėse duomenų bazėse: PubMed, ScienceDirect ir Cochrane Library, panaudojus filtrus, raktažodžių kombinacijas ir pritaikius įtraukimo/atmetimo kriterijus. Tyrimuose pacientams turėjo būti nustatyta obstrukcinė miego apnėja ar obstrukcinės miego apnėjos sindromas (OMAS) ir atlikta abiejų žandikaulių ortognatinė operacija: Le Fort I osteotomija viršutiniam žandikauliui, abipusė sagitalinio skėlimo osteotomija su ar be smakro osteotomijos apatiniam žandikauliui, vertintos apnėjų–hipopnėjų indekso (AHI) ir Epworth mieguistumo skalės (EMS) reikšmės prieš ir po ortognatinės operacijos. Atrinktų publikacijų kokybė įvertinta Newcastle–Ottawa įrankiu. Paskutinė straipsnių paieškos data: 2025 m. vasario 16 d. Rezultatai. Į mokslinę literatūros apžvalgą įtraukti 6 tyrimai, 2 iš jų kohortiniai retrospektyviniai, 4 – perspektyviniai. 5 straipsniai priskirti aukštos ir 1 vidutinės kokybės tyrimams. Iš viso tyrimuose dalyvavo 162 OMA/OMAS sergantys pacientai, kuriems atlikta abiejų žandikaulių ortognatinė operacija. Atlikus kokybinę įtrauktų publikacijų tyrimų rezultatų analizę, gautas reikšmingas AHI ir EMS rodiklių sumažėjimas (p < 0,05) visose analizuotose publikacijose. Po ortognatinės operacijos OMA sunkumo forma sumažėja nuo sunkios ar vidutinės iki vidutinės ar lengvos arba liga visai išnyksta, kartu mažėjant mieguistumui dienos metu. Išvados. Ortognatinė operacija yra efektyvus gydymo metodas pacientams, sergantiems obstrukcinės miego apnėjos forma, remiantis apnėjos–hipopnėjos indeksu ir Epworth dienos mieguistumo vertinimo skale.
