Abstract
Erzurum Şehir Hastanesi tarafındanyürütülmekte olan ceza soruşturması kapsamında Bilirkişi raporu talep edilmesiüzerine ‘Hasta; 10468728286 T.C. numaralı Esin Akal’a ait dosya tarafımızcaincelendi. Dosyada; genel durumu iyi, şuuru açık ve GKS:15 olduğu yakınlarıtarafından belirtilen 72 yaşındaki, HT ve AF tanıları mevcut bayan hasta17.04.2025 tarihinde lumbal disk hernisi nedeniyle hasta operasyona alınmış.ETGA altında L2-3-4-5 vertebralarına cerrahi işlem uygulanmış. Hasta extübasyoniçin pron pozisyonundan çevrilmiş. Tansiyonu yüksek seyreden hastaya gereklimüdahaleler anestezi kliniğince yapıldıktan sonra extübe edilmiş. Tansiyonlarıyine yüksek seyreden ve saturasyonunda düşme gözlenmesi üzerine hasta reentübeedilmiş. Tekrar extübasyonu denenen hastanın bu kez nöbet geçirince veanizokorisi olduğu gözlenince BBT çekilip Beyin cerrahisi yoğunbakım ünitesine devredilmiş. 18.04.2025 tarihinde çekilen BBT vedifüzyon MR sonuçlarına göre bazal ganglionlarda hipoksi ile uyumlu olabilecekdansite artışı izlenmiş. 09.05.2025tarihinde Geriartri Ünitesine oradan da nöroloji servisine devredilmiştir. Enson 10.06.2025 tarihinde Fizik Tedavi Ünitesine yatırılıp ardından taburcuedilmiştir. Hasta yakınları hastanın operasyon sonrası belirgin şuur geriliği,her iki gözde görme kaybı bulunduğunu ve vücudunun sağ tarafında belirgin güçkaybı geliştiğini idda etmektedir.
