Management of peri-implant soft tissues in second stage surgery: a systematic review and meta-analysis

Abstract
Declaración del problema: Un momento clave en la rehabilitación sobre implantes es el manejo del fenotipo periimplantar a través de los tejidos blandos en la segunda fase quirúrgica. Objetivo: Evaluar los resultados clínicos obtenidos con las diferentes técnicas de cirugía plástica periimplantaria de segunda fase para mejorar el ancho de tejido queratinizado y grosor de tejido blando. Materiales y métodos: Esta revisión se realizó de acuerdo con la guía PRISMA 2020. Los estudios se buscaron, seleccionaron y analizaron en tres bases de datos MEDLINE, EMBASE, Cochrane y en revistas de alto impacto en periodoncia e implantología obteniendo un total de 23 artículos incluidos. La evaluación de la calidad de los artículos se realizó usando New Casttle Ottawa, riesgo de sesgo y la herramienta JBI para series de caso. Se realizó un metaanálisis de medias únicas y fue resumido como efectos medios ponderados (EMP) para evaluar el ancho de tejido queratinizado (ATQ) y grosor del tejido blando (GTB). Resultados: Para ganancia de ancho de tejido queratinizado la mayor ganancia en mm fue el colgajo posicionado apicalmente (CPA) con injerto gingival libre (IGL), matriz de colágeno xenogénico (MCX), injerto de tejido conectivo (ITC) y fibrina rica en plaquetas (FRP). Con respecto al aumento de grosor de tejidos blandos los mejores resultados fueron con la técnica de colgajo de espesor parcial/total (CEP/T) con ITC o MCX. Conclusiones: La mejor técnica para aumentar el tejido queratinizado alrededor de los implantes dentales es el colgajo posicionado apicalmente con injerto gingival libre, el segundo con mayor ganancia en mm es el colgajo posicionado apicalmente con matriz de colágeno xenogénico. Para el aumento del grosor de los tejidos blandos periimplantarios, la mejor técnica es el colgajo de espesor parcial/total con injerto de tejido conectivo, el segundo con mayor ganancia en mm es el colgajo de espesor parcial/total con matriz de colágeno xenogénico.
