Alternative Management in Surgical Treatment of Knee Osteoarthritis: Early Results of Our Patients with Proximal Partial Fibular Resection

Abstract
Diz osteoartriti (OA), eklem ağrısı, sertlik ve deformitenin eşlik ettiği kronik, progresif bir dejeneratif hastalıktır. Diz osteoartriti 60 yaşından büyük popülasyonun %30'unda görülme sıklığı olan yaygın bir eklem hastalığıdır.Diz osteoartriti olan hastalarda cerrahi tedavi seçeneği olarak yüksek tibial osteotomi, unikompartmantal veya total diz protezi uygulanabilmektedir. Günümüzde yeni bir teknik olarak, yüksek tibial osteotomi ve total diz protezine oranla daha kolay ve daha az invaziv bir cerrahi tedavi yöntemi olan proksimal parsiyel fibular rezeksiyon gündeme gelmiştir. Çalışmamızdaki amacımız; proksimal parsiyel fibular rezeksiyon (PPFR) hastalarının preoperatif ve postoperatif 6. aydaki bacak uzunluk grafisi üzerinden hesaplanan femorotibial açı değişimini ve lateral kompartman yükseklik değişimini değerlendirmek, diz skorlarını VAS (Vizüel Analog Skor) ve WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index) karşılaştırmak ve yöntemin etkinliğini göstermektir. Ocak 2017 ile Aralık 2017 arasında opere edilen hastalar retrospektif olarak tarandı. PPFR uygulanan 18 hasta (7 hasta erkek, 11 hasta kadın) çalışmaya dahil edildi. Hastaların ortalama takip süresi 8.2 ay (6-11 ay) olarak belirlendi. Ortalama yaş 66.2 (56-72 yaş) idi. PPFR yapılan hastalarda postoperatif dönemde, diz eklem ağrısında azalma, varus diziliminde düzelme gözlenmiştir. Klinik ve radyolojik parametrelerde; preoperatif değerlerle karşılaştırıldığında postoperatif değişim istatistiksel olarak anlamlı bulunmuştur. Sonuç olarak; PPFR, varus osteoartritindeki dizde ağrıyı önemli ölçüde azaltabileceği, diz ekleminin radyografik görünümünü iyileştireceği, total diz artroplastisine olan ihtiyacı geciktirebileceği, postoperatif dönemde rehabilitasyonu kolaylaştırabileceği kanaatindeyiz.
