Amaç: Pes ekinovarus (PEV) doğuştan gelen bebeklerin ayak postürünü etkileyen ortopedik bir problemdir. Ponseti yöntemi literatürde PEV tedavisinde etkinliği gösterilmiş bir yöntemdir. Bebekler doğumdan sonra seri alçılama, cerrahi ve sonrasında breys kullanımını içeren sistematik bir tedavi programı ile takip edilir. Dennis Brown ateli (DBA) tedavi sürecinde kullanılan geleneksel ortezdir. Ancak günümüzde addüksiyon dorsifleksiyon mekanizması (ADM) gibi modern cihazların kullanımı yaygınlaşmıştır. Bu çalışmanın amacı PEV tanılı çocuklarda alçılama ve cerrahi sonrası kullanılan ADM ve DBA breyslerinin kullanım zorlukları, maliyetleri ve etkinliğinin aile perspektifinden değerlendirilmesi ve ailelerin cihaz memnuniyetlerinin karşılaştırılmasıdır. Yöntem: Çalışmaya 3 yaş altında kliniğimizde PEV tedavisi sürecinde DBA (n=16) ve ADM (n=12) cihazları kullanan toplam 28 çocuk dahil edilmiştir. Olguların aileleri telefonla aranıp demografik bilgiler, cihaz kullanım süreci, karşılaşılan zorluklar, cihaza uyum, tedavinin etkinliği ve ailelerin cihaz maliyet algısı numerik skala ile değerlendirilmiştir. Cihaz memnuniyeti de Quebec yardımcı teknoloji memnuniyet anketi ile değerlendirilmiştir. Gruplar arasındaki fark Mann Whitney-U testi ile karşılaştırılmıştır. Bulgular: Gruplar arası karşılaştırmalarda ADM grubunda çocuğun cihaz uyumu (ADM:7.5±2.9; DBA:5.8±2.1; U=143.5, p=0.027), cihaz maliyeti algısı (ADM:7.4±2.8; DBA:3.1±1.9; U=170.0, p=0.001), etkinin büyüklüğü (ADM:4.8±3.3; DBA:1.6±3.2; U=142.0, p=0.024) ve Quebec skorunun (ADM:4.3±0.6; DBA:3.5±0.6; U=160.0, p=0.003) anlamlı derecede daha yüksek olduğu bulunmuştur. Diğer değişkenlerde (cihaz kullanım zorluğu, cihaz memnuniyeti, fayda, aile için zorluk, tedavi memnuniyeti) istatistiksel fark saptanmamıştır. Sonuç: ADM grubunda memnuniyet ve gelişim etkisi daha yüksek bulunurken, DBA grubu maliyet avantajı sağlamaktadır. Çalışmanın sonuçları ADM cihazlarının hasta uyumu ve genel memnuniyet açısından üstün olduğunu, DBA cihazlarının ise ekonomik açıdan avantajlı olduğunu göstermektedir.
