Clinical spectrum of neuropathic symptoms associated with impaired glucose tolerance

Abstract
Viena iš dažniausių idiopatinės polineuropatijos priežasčių yra gliukozės toleravimo sutrikimas (GTS). Periferinių nervų pažaida GTS metu dažnai neįtariama ir nediagnozuojama, neatsižvelgiant į jos didelį paplitimą tarp idiopatinės neuropatijos atvejų ir panašumą į ankstyvo cukrinio diabeto metu atsiradusią skausmingą polineuropatiją. Tyrimo tikslas. Nustatyti ir įvertinti su GTS susijusios polineuropatijos klinikinius simptomus ir skausmo parametrus. Tiriamieji ir metodai. Tyrime dalyvavo vyresni nei 30 m. vyrai ir moterys, kuriems nustatytas GTS (n = 47). Subjektyvūs neuropatiniai simptomai vertinti pagal atsakymus į Mičigano neuropatinių simptomų klausimyną. Patiriamo skausmo intensyvumui galūnėse vertinti naudota skaičių analogijos skalė (SAS). Kokybiniai skausmo parametrai vertinti taikant lietuvišką A. Pakulos sudarytą McGill skausmažodžių klausimyno analogą ir neuropatinio skausmo klausimyną (NSK). Klinikinio neurologinio ištyrimo metu vertinta raumenų jėga, sausgyslių refleksai, lietimo, skausmingo adatos dūrio, temperatūros, vibracinis ir propriorecepcinis jutimai kojose. Rezultatai. Pagal pacientų atsakymus į Mičigano neuropatinių simptomų klausimyną nustatyta, kad dauguma jų patyrė neuropatinius simptomus kojose ar pėdose. Daugiau nei 60 % tiriamųjų patyrė deginantį skausmą. GTS pacientai pagal SAS jautė silpno ir vidutinio stiprumo skausmą, susidedantį iš gilaus ir paviršinio skausmo parametrų. NSK skausmo skalių suma sudarė 30,81 ± 20,66 (p < 0,05). Vidutinis sensorinio skausmo kokybinis indeksas pagal Pakula-McGill skausmažodžių klausimyną buvo 30,06 ± 11,50 balo, emocinio – 13,39 ± 7,00 balai. Skausmas buvo lėtinis 77,3 % pacientų. [...].
