The Effects of Cannabis Reduction on Clinical Symptoms in People with Comorbid Posttraumatic Stress Disorder and Cannabis Use Disorder

Abstract
Objective: To investigate the effects of cannabis reduction on posttraumatic stress disorder (PTSD) outcomes in patients with comorbid PTSD and cannabis use disorder. Methods: Participants attempted 12 weeks of cannabis abstinence. Abstinence was promoted through weekly support sessions and contingent cash bonuses. Participants were entitled to an increasing cash bonus if abstinence was biochemically verified via urine toxicology at weeks four, eight, or 12. Results: 24 individuals were enrolled, and 14 completed the 12-week study. We established a positive within-subject association between cannabis consumption and PTSD outcomes. We found significantly greater improvements in symptoms, severity, avoidance, and hyperarousal from baseline to week 12 among week-12-abstainers relative to non-abstainers. Conclusions: We provide preliminary evidence to suggest that cannabis consumption is positively associated with PTSD outcomes. Extended periods of abstinence may be associated with greater reductions in certain PTSD outcomes. A larger sample size is needed to confirm these results.
