Understanding the time to switch from conventional systemic to biologic therapy for the treatment of psoriasis and psoriatic arthritis in Ontario, Canada

Abstract
Psoriasis (PsO) is a chronic inflammatory immune-mediated disease affecting the skin and joints. Approximately 1-in-3 patients with PsO develop Psoriatic Arthritis (PsA). This longitudinal retrospective cohort study aimed to investigate the mean time to biologic (TTB) initiation among adults aged 66+ diagnosed with PsO and PsA in Ontario, and determine patient and prescriber characteristics associated with TTB. Among 10,455 patients initiating a conventional treatment, the mean (SD) TTB was 989 days (1,019.9), and 13 patient and prescriber characteristics were associated with TTB. Overall, patients aged 75+, with high deprivation marginalization, moderate residential instability, methotrexate for index medication, dementia, cancer, and prescribed by physicians 46+ had a slower TTB. Patients diagnosed with concurrent PsO and PsA, IBD, prescribed by a female physician, rheumatologist, dermatologist, or internal medicine physician had a faster TTB. Further research is warranted to explore the impact of these factors on long-term treatment outcomes and healthcare resource utilization.
