Abstract
Primary hyperparathyroidism (PHPT), a prevalent endocrine disorder, significantly impacts the skeletal, renal, cardiovascular, and neurocognitive systems, with surgical parathyroidectomy (PTx) serving as the only definitive treatment. Despite the availability of safe and effective treatment, PHPT remains underdiagnosed and undertreated, necessitating further investigation into the benefits of PTx. This thesis utilized a cohort of biochemically diagnosed PHPT patients to evaluate outcomes associated with surgery. The findings revealed a 22% reduction in the hazard of any fracture, with both immediate and long-term benefits observed. We also identified a 20% decrease in the risk of any cardiovascular event compared to non-operative management. Specific cardiovascular benefits included reduced risks of ischemic stroke, acute myocardial infarction, atrial fibrillation, and congestive heart failure. These findings underscore the potential for broadening the clinical application of PTx to improve patient outcomes, mitigate complications, and guide future population health strategies.
