In the United States, calcium channel blockers were involved in at least 11,764 poisonings and 78 deaths in 2011. Unfortunately, there is an important practice variation and inconsistency in the management of this poisoning that may lead to a possible care gap when clinicians do not adhere to recommendations supported by the evidence. The goal of this thesis is to build an evidence-based guideline for the treatment of calcium channel blocker poisoning endorsed by international organizations. In doing so, this research may substantially improve consistency and adherence to recommendations by adapting the evidence outlined by our systematic review to costs, resources, values and preferences. Following a review of the literature concerning the mechanisms of toxicity, the clinical presentation, the rational for the use of different interventions, and the theory of guideline development, the research program detailed in this thesis stresses the need for a guideline by showing inconsistency with the current poison control recommendations and by demonstrating the burden of illness in a retrospective study. Moreover, an important practice variation was underlined in a survey of emergency physicians. In order to develop an evidence-based guideline, the information required for decision making is then collected in a systematic review describing the low to very low level of evidence for each intervention, and a cost-effectiveness analysis confirming that cost should not limit access to invasive strategy such as VA-ECMO in centers where the intervention is available. After conducting a rigorous development strategy, an international group of experts in emergency medicine, critical care and toxicology decided that there was insufficient evidence upon which to create a guideline and reached a concensus opinion to create recommendations for the treatment of calcium channel blocker poisoning. The treatment recommendations would encourage consistency in messaging to the clinician based on the poor level of evidence, the balance between risks and benefits, costs and resources, and values and preferences. This thesis contributes to the development of recommendations for the in-hospital management of calcium channel blocker poisoning. It presents reflections on the process of guideline development in acute care toxicology and suggests future directions to consider. Il existe malheureusement une variation importante en ce qui a trait à la manière de prendre en charge ce type d'intoxication. Lorsque les recommandations basées sur les évidences ne sont pas respectées, cela peut se traduire par des soins sub-optimaux aux patients. Le but de cette thèse est de développer un guide de pratique clinique basé sur les évidences afin de prendre en charge les intoxications par les bloqueurs des canaux calciques. Le programme de recherche qui suit peut contribuer de façon susbtantielle à améliorer la conformité aux recommandations en adaptant les données probantes soulignées par notre revue systématique aux coûts, aux ressources disponibles ainsi qu'aux valeurs et préférences des décideurs, des utilisateurs du guide, des patients et de leur proches. La présente thèse débute par une revue de littérature détaillant les mécanismes de toxicité de l'intoxication par les bloqueurs des canaux calciques, la présentation clinique, la rationnelle pour l'utilisation des diverses interventions et la théorie concernant le développement des guides de pratique. Le programme de recherche souligne ensuite l'importance de développer un guide de pratique clinique pour ce type d'intoxication. En effet, une étude rétrospective démontre le fardeau relié à cette condition et une enquête souligne l'importante variation dans les pratiques médicales. Afin de colliger les informations requises pour la prise de décision lors du développement du guide de pratique clinique, une revue systématique décrivant les degrés d'évidence pour chaque intervention a été complétée. Aussi, une étude coûts-efficacité a été conduite afin de confirmer que les coûts ne devraient pas être une raison pour limiter l'accès à la circulation extracorporelle dans les centres où l'intervention est disponible. Après avoir mis sur pied une méthodologie rigoureuse, un groupe international d'experts en médecine d'urgence, en soins intensifs et en toxicologie a jugé que les données probantes étaient insuffisantes pour développer un guide de pratique clinique. Il a donc établi un concensus pour émettre des recommandations pour le traitement de l'intoxication par les bloqueurs des canaux calciques. Ces recommandations favoriseront une meilleure cohérence en ce qui a trait aux messages transmis aux cliniciens tout en se basant sur le faible degré d'évidence, l'évaluation de l'équilibre entre les risques et bénéfices, les coûts, l'utilisation des ressources, les valeurs et préférences. Cette thèse contribue au développement de recommandations pour la prise en charge intra-hospitalière de l'intoxication par les bloqueurs des canaux calciques. Elle constitue une réflexion sur le processus de création des guides de pratique cliniques en toxicologie de soins aigus et suggère de futures avenues à considérer.
