Development, Validation, and Implementation of a Clinical Decision Tool for Blood Pressure Management in Patients on Hemodialysis

Abstract
Hypertension is a strong risk factor for cardiovascular disease and remains uncontrolled in a large proportion of patients on hemodialysis. Management requires a multifactorial approach involving optimization of hemodialysis specific factors and medications. The objective of this study was to develop, validate, and implement a clinical decision tool to help improve blood pressure control for patients on hemodialysis. A clinical decision tool was developed and assessed for face and content validity. The tool was then implemented in a cohort of patients on hemodialysis. Blood pressure control was similar pre and post implementation and without a greater incidence in adverse events compared to the control group. A number of blood pressure related interventions were made including the discontinuation and dose reduction of antihypertensive medications. Implementation of the tool in a larger cohort of patients for an extended time period is required to further assess implications of the tool in clinical practice.
