Immunopathology of chikungunya virus infection: Potential biological markers for predicting the risk of chronic sequelae

Introducción: La infección por el virus chikungunya (CHIKV) representa un importante problema de salud pública debido a la cronificación de síntomas reumatológicos, que deterioran la calidad de vida relacionada con salud (CVRS) y aumentan la fatiga tras la fase aguda. Sin embargo, los mecanismos patogénicos de la enfermedad no están completamente definidos y aún se carece de biomarcadores pronósticos validados para la identificación temprana de artropatía crónica. Objetivo: Identificar biomarcadores para predecir la evolución hacia secuelas crónicas (reumatismo crónico post-CHIKV - RC-pCHIK), fatiga crónica y CVRS afectada, en pacientes con enfermedad por CHIKV. Metodología: Se realizó una revisión sistemática de estudios publicados que evaluaron biomarcadores inmunológicos en la infección por CHIKV. Además, se desarrolló un estudio de cohorte con seguimiento a largo plazo de personas infectadas durante el brote epidémico de 2014–2015 en Piedecuesta, Santander. Finalmente, en dos estudios de cohorte se exploró la asociación entre biomarcadores inmunológicos y el desarrollo de RC-pCHIK. Resultados: La revisión sistemática incluyó 38 estudios e identificó un perfil inmune inflamatorio agudo con elevación de factores inmunológicos, sin lograr establecer biomarcadores pronósticos consistentes. En la cohorte, se identificó que el 14,1% de los participantes fueron clasificados como casos de RC-pCHIK, quienes presentaron mayor afectación de la CVRS y fatiga crónica. En el estudio de casos y controles, se encontró que niveles más altos de IL-8/CXCL8 e IP-10/CXCL10 y MIG/CXCL9 medidos en fases tempranas de la infección se asociaron con una menor probabilidad de desarrollar RC-pCHIK. Conclusión: Estas quimiocinas podrían ser útiles para estratificar el riesgo de progresión hacia secuelas de reumatológicas tras una infección por CHIKV. Sin, embargo, no se observó una asociación con CVRS afectada ni fatiga crónica. Estos hallazgos deben ser validados en estudios adicionales.
