Investigating the Link Between Intrarenal Complement Expression and Proteinuric Chronic Kidney Disease

Abstract
Chronic kidney disease (CKD) is a major healthcare challenge. Evidence suggests that thecomplement system may represent a key shared pathway involved in proteinuric CKD progression, regardless of the underlying cause. Studies suggest that during proteinuria, circulating complement proteins leak into the tubular space, causing tubular damage. Additionally, it is speculated that during proteinuria, tubular cells synthesize complement proteins that become locally activated, further contributing to tubular pathology. Therefore, we aimed to explore the link between complement, specifically intrarenal complement expression, and proteinuric CKD. We hypothesized that intrarenal complement expression is a key shared mechanism involved in the progression of proteinuric CKDs. We demonstrated constitutive renal expressions of complement transcripts, with upregulation in patients with various proteinuric CKDs. Additionally, we showed correlation between tubular complement expressions and poor renal outcomes. Ultimately, we demonstrated a link between complement and proteinuric CKDs, thus supporting complement as a potential therapeutic target.
