A cárie dentária é uma das doenças crónicas mais prevalentes no mundo, que envolve danos no esmalte dentário e com etiologia multifatorial envolvendo fatores microbianos, dietéticos, comportamentais e genéticos. A β-defensina 1 (hBD‑1) é um peptídeo antimicrobiano codificado pelo gene DEFB1 e é a principal molécula de proteção contra a cárie dentária, estando envolvida na defesa imunológica da cavidade oral. O gene DEFB1, responsável pela produção da hBD‑1, apresenta polimorfismos genéticos que podem influenciar a sua expressão e, consequentemente, a suscetibilidade à cárie. O objetivo desta revisão sistemática foi averiguar na literatura científica que polimorfismos do gene DEFB1 podem apresentar alguma influência na suscetibilidade à cárie dentária. A metodologia envolveu a procura sistematizada nas bases PubMed, Science Direct e Web of Science, com inclusão de estudos originais publicados entre 2014 e 2024, que analisaram a relação entre os polimorfismos rs11362, rs1799946, rs1800972 e rs1047031 e a ocorrência de cárie. Onze estudos atenderam aos critérios de inclusão, envolvendo populações de diversos países e faixas etárias. A maioria dos estudos revelou associação significativa entre o polimorfismo rs11362 e o aumento da suscetibilidade à cárie, especialmente em crianças. Além disso, alguns estudos demonstraram que os genótipos de risco (rs11362, rs1800972) estavam associados a níveis reduzidos de expressão da hBD‑1, indicando um possível mecanismo funcional. A qualidade metodológica foi avaliada através da escala Newcastle-Ottawa (NOS), evidenciando confiabilidade razoável a boa nos estudos incluídos. Conclui-se que os polimorfismos do gene DEFB1, particularmente o rs11362, estão associados à suscetibilidade à cárie dentária em diversas populações. A identificação desses marcadores genéticos pode contribuir para o desenvolvimento de estratégias de prevenção personalizadas baseadas no risco genético individual, além de fomentar novas abordagens terapêuticas voltadas à modulação da resposta imune oral.
