Mục tiêu: Nghiên cứu giá trị của chụp cắt lớp vi tính (CLVT) đa dãy đầu thu trong chẩn đoán giai đoạn bệnh và tiên lượng khả năng phẫu thuật u vùng đầu tụy. Đối tượng và phương pháp: Tiến cứu, mô tả cắt ngang được tiến hành trên 79 bệnh nhân nghi ngờ u vùng đầu tụy trên lâm sàng, được chụp CLVT 320 dãy, điều trị phẫu thuật và có kết quả mô bệnh học tại Bệnh viện Trung ương Quân đội 108, từ tháng 05/2017 đến tháng 02/2021. Kết quả: Độ chính xác của CLVT trong chẩn đoán u vùng đầu tụy là 93,7%. CLVT phù hợp cao với mô bệnh học trong chẩn đoán vị trí u vùng đầu tụy với hệ số Kappa là 0,73. CLVT phù hợp rất cao với phẫu thuật trong đánh giá xâm lấn mạch máu của u vùng đầu tụy (hệ số Kappa: 0,871). CLVT phù hợp vừa với mô bệnh học trong đánh giá xâm lấn tại chỗ của u vùng đầu tụy (hệ số Kappa: 0,423). Chẩn đoán di căn hạch và giai đoạn bệnh của CLVT phù hợp vừa với mô bệnh học, hệ số Kappa lần lượt là 0,417 và 0,405. Kết luận: Qua nghiên cứu có đối chiếu với phẫu thuật và giải phẫu bệnh, chụp cắt lớp vi tính có giá trị cao trong việc chẩn đoán giai đoạn bệnh và tiên lượng khả năng phẫu thuật u vùng đầu tụy.
