Abstract
Mục tiêu: Khảo sát tỷ lệ suy yếu theo thang điểm suy yếu lâm sàng của Canada (Clinical Frail Scale - CFS) và mối liên quan với các biến cố bất lợi tại bệnh viện ở người cao tuổi mắc hội chứng động mạch vành cấp. Đối tượng và phương pháp: Nghiên cứu tiến cứu, quan sát, tiến hành trong thời gian từ tháng 02/2021 đến tháng 09/2022, tại Viện Tim mạch Việt Nam, Bệnh viện Bạch Mai. Bệnh nhân trên 60 tuổi, được chẩn đoán hội chứng động mạch vành cấp, được đánh giá tình trạng suy yếu theo thang điểm CFS và theo dõi các biến cố bất lợi trong thời gian nằm viện. Các biến cố bất lợi tại Bệnh viện bao gồm: Tử vong, tai biến mạch não, viêm phổi mắc phải, rối loạn nhịp tim, xuất huyết nặng. Các biến cố lâm sàng thực (Net adverse clinical events - NACE) được định nghĩa là biến gộp của tử vong, tai biến mạch não và xuất huyết nặng. Kết quả: 145 bệnh nhân, độ tuổi trung bình: 72,68 ± 6,28. Tỷ lệ suy yếu là 36,55%. Các bệnh đi kèm phổ biến nhất là tăng huyết áp (80,0%), đái tháo đường (33,10%), suy tim (15,86%), rối loạn lipid máu (13,10%) và tiền sử đột quỵ trước đó (10,34%). So với nhóm bệnh nhân cao tuổi không suy yếu, nhóm cao tuổi suy yếu có nguy cơ cao hơn đáng kể về biến cố bất lợi tại bệnh viện (OR = 2,32; CI 95%: 1,08-5,01) và biến cố lâm sàng thực (OR = 5,27; CI 95%: 1,28-21,65). Kết luận: Tình trạng suy yếu khá thường gặp, chiếm 36,55% và là một yếu tố nguy cơ đáng kể dẫn đến gia tăng các biến cố bất lợi ở người bệnh cao tuổi mắc hội chứng động mạch vành cấp.
