Ingesta dietética de flavonoides y la incidencia de Alzheimer y deterioro cognitivo en adultos: revisión sistemática

Fundamentos: Los flavonoides son antioxidantes y antiinflamatorios que protegen las neuronas, mejoran la función sináptica y previenen la apoptosis, favoreciendo la salud cerebral y podrían prevenir el deterioro cognitivo. El objetivo fue sistematizar la literatura que evalúa la asociación entre el consumo de flavonoides en la dieta y la incidencia de demencia en adultos mayores de 45 años. Métodos: Se realizó una revisión sistemática de 13 estudios de cohorte publicados hasta abril de 2022 en Embase, PubMed, Scopus y Scielo. Se incluyeron estudios que evaluaron la ingesta dietética de flavonoides mediante cuestionarios validados, con desenlaces como deterioro cognitivo y Alzheimer. Los estudios fueron evaluados con la escala Newcastle-Ottawa, obteniendo puntuaciones de 6 a 9 puntos. Resultados: De un total de 1.525 artículos, se incluyeron 13 estudios de cohorte que abarcaron a 41.229 participantes de entre 45 y 98 años. Los resultados establecieron asociaciones significativas entre un alto consumo de flavonoides, en particular flavonoles y antocianinas, y un menor riesgo de deterioro cognitivo y Alzheimer en dos estudios que presentaron una alta ca­lidad metodológica. No obstante, la heterogeneidad en los métodos de evaluación y los sesgos asociados a los cuestionarios dietéticos dificultaron la comparabilidad. Asimismo, la falta de análisis de biomarcadores y la aplicación inconsistente de herramientas cognitivas podrían limitar la validez de los hallazgos. Conclusión: Los flavonoides podrían tener un efecto protector frente a la demencia. Se sugiere que investigaciones futuras empleen diseños más homogéneos, incluyan biomarcadores y mejoren el control de factores confusores para fortalecer la base de evidencia.
