在宅要介護高齢者の転倒要因についての考察―転倒予防自己効力感と注意・遂行機能に着目して―

【はじめに、目的】高齢者の転倒は外傷などの身体的な問題だけではなく、再転倒への恐怖心などの心理的問題も生じさせる。転倒恐怖感を測定する自己効力感尺度として「転倒予防自己効力感尺度(Fall-Prevention Self Efficacy Scale :FPSE)」があるが、転倒の予測因子とはなっていない。一方、高齢者の認知機能状態は転倒リスクを増大させる要因であり、認知機能が低下している者は注意・遂行機能が低下している。本研究は在宅要介護高齢者の転倒要因について、FPSEや注意・遂行機能に着目して考察することを目的とした。 【方法】A訪問看護ステーションを介護保険にて利用している高齢者のうち、屋内の移動が自立している者・認知機能の著明な低下がない者・転倒状況が確認できる者・住宅環境に問題がない者・転倒に影響を及ぼす薬剤を使用していない者を対象とした。対象者のFPSE・Trail Making Test Part A(TMT-A)・日本語版Montreal Cognitive Assessment(MoCA-J)を測定し、測定終了から6か月間の転倒について調査した。期間終了後、対象者を転倒群と非転倒群に群分けし、各群間でのFPSE・TMT-A・ MoCA-Jの差をマン・ホイットニーのU検定にて算出した。統計処理はIBM SPSS Statisticsを用い、統計的有意水準を5%とした。また、転倒者の転倒要因について、転倒時の状況や各群間の差から考察を行った。 【結果】対象者は26名(平均年齢82.8±7.7歳、女性14名、転倒者4名)となった。転倒群の各測定値の平均はFPSE29.5±2.5、TMT-A240.0±72.8、MoCA-J15.8±2.8、非転倒群ではFPSE22.2±6.8、TMT-A128.4±36.5、MoCA-J22.8±3.8であった。FPSE・TMT-Aは転倒群が有意に高く、MoCA-Jは転倒群が有意に低かった。転倒者の転倒時の状況はA「自宅内で小走りをして転倒した」、B「外出時は使用している歩行器を使用せずに屋外を歩き転倒した」、C「台風接近時に外出、風に煽られて転倒した」、D「お茶が入っている湯呑を持ちながら歩き転倒した」であった。 【考察】転倒群は注意・遂行機能や認知機能が低下しているにもかかわらず、転倒予防自己効力感が高く、転倒に対する恐怖感が弱いという結果となった。転倒した4名は普段の歩行・動作以上の難度の動作を行い、転倒する傾向があった。認知機能や注意・遂行機能の低下は、身体機能の自己認識の逸脱を生じさせる。この自己認識の逸脱が、過信・転倒恐怖感の減衰を招くことでFPSEが増加し、自身の能力以上の動作を行うことで転倒を生じさせたと考えられた。 【結果】本研究では認知機能や注意遂行機能の低下が転倒に対する恐怖感を減衰させ、高齢者の転倒を生じさせる可能性が示唆された
