Acupuncture Methods Efficacy in Patients with Knee or Hip Osteoartritis

Abstract
Įvadas: Klubo ir kelio sąnarių skausmas bei funkcijos sutrikimas yra dažni osteoartrito (OA) požymiai. Suprastėjus judėjimo funkcijai bei jaučiant nuolatinį skausmą formuojasi procesai, neleidžiantys asmeniui pilnavertiškai funkcionuoti kasdieninėje veikloje. Šiuolaikinėje medicinos praktikoje reabilitacijos metodų taikymas sergant kelio ar klubo sąnario osteoartritu yra neatsiejama gydymo dalis. Viena iš galimų reabilitacijos priemonių gydant osteoartritą,– akupunktūra, kuria siekiama sumažinti pacientų patiriamą skausmą. Tikslas: Įvertinti akupunktūros metodų efektyvumą pacientams, sergantiems kelio ar klubo sąnario osteoartritu. Uždaviniai: 1. Nustatyti akupunktūros metodų efektyvumą skausmo mažinimui pacientams, sergantiems kelio ar klubo sąnario osteoartritu. 2. Įvertinti ar akupunktūros metodų taikymas efektyvesnis skausmo mažinimui pacientams, prieš totalinę artroplastiką, ją taikant kartu su konservatyviu gydymu, ar juos taikant pacientams po totalinės artroplastikos. Metodai: Atlikta išsami paieška naudojant šias duomenų bazes: MEDLINE per PubMed siekiant atrinkti klinikinius atsitiktinių imčių tyrimus nuo 2010 m. iki 2019 gruodžio mėnesio imtinai, kuriuose akupunktūros gydymo metodai buvo naudojami kelio ir/arba klubo sąnario osteoartrito (OA) gydymui siekiant sumažinti skausmą. Tyrimai analizei buvo atrinkti pagal rezultatų vertinimo sistemą: vizualią analogijos skalę (VAS) arba Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Rezultatai: Kiekybinė sisteminė literatūros apžvalga remiasi 10 atliktų klinikinių tyrimų rezultatais, kuriuose buvo stebimas teigiamas skausmo pokyčio sumažėjimas po taikytos procedūros į akupunktūrinius taškus. Vidutiniškai visiems tiriamiesiems buvo nustatytas 44,1 proc. skausmo sumažėjimas pagal VAS ir apie 31 proc. pagal WOMAC. Vidutiniškai visiems tiriamiesiems skausmas, kai akupunktūros būdai buvo taikomi be chirurginio gydymo sumažėjo apie 41,1 proc., o po taikytos artroplastikos - apie 48 proc. Išvados: Akupunktūros gydymo metodas yra statistiškai reikšmingas (p 0,05) ir kliniškai efektyvus skausmo mažinimui pacientams, sergantiems kelio ar klubo sąnario osteoartritu. Tačiau teigti, kad yra efektyvesnis taikant kartu su konservatyviu gydymu ar po totalinės klubo ar kelio sąnario artroplastikos negalima.
