Однак попри їх широке використання в клінічній практиці, клінічне значення цих показників у перебігу COVID-19-асоційованої пневмонії залишаються не до кінця з’ясованими. Метою дослідження було обґрунтування внеску клінічних показників крові у хворих з нюховою дисфункцією для діагностики та прогнозування перебігу SARS-CoV-2-асоційованої пневмонії різного ступеня тяжкості. Ретроспективний аналіз охопив 370 історій хвороби пацієнтів, які проходили лікування в центрі надання медичної допомоги хворим на COVID-19 м. Харкова. Аналіз включав оцінку клінічного перебігу пневмонії, наявності нюхових та загальних отоларингологічних симптомів, а також рівня окремих лабораторних показників крові. Статистичний аналіз включав проведення дискримінантного аналізу з розрахунком нормованої евклідової відстані для визначення діагностичної значущості клінічних показників крові. Аналіз даних показав, що 52,2 % пацієнтів відзначали виникнення нюхової дисфункції, яка найчастіше реєструвалася у хворих із легким перебігом SARS-CoV-2-асоційованої пневмонії (68,1 %) та у пацієнтів без пневмонії (88,6 %). Статистичний аналіз показав, що вивчення показників клінічного аналізу крові було доцільним у пацієнтів із відносно легкими формами COVID-19, оскільки це дозволило знизити ймовірність діагностичних помилок до 0,03. У хворих із середнім та тяжким перебігом SARS-CoV-2-асоційованої пневмонії специфічні показники COVID-19 уже забезпечують високу точність діагностики (похибка менше ніж 0,02), а включення параметрів клінічного аналізу крові лише підвищує точність моделі дискримінації. Таким чином, нюхові порушення, асоційовані з гострим риносинуситом при COVID-19, можуть слугувати прогностичним маркером більш сприятливого перебігу пневмонії у госпіталізованих пацієнтів. Водночас показники клінічного аналізу крові мають обмежену додаткову цінність у розширеній діагностиці. However, despite their widespread use in clinical practice, the clinical significance of these indicators in the course of COVID-19-associated pneumonia remains unclear. The aim of the study was to substantiate the contribution of clinical blood parameters in patients with olfactory dysfunction in the diagnosis and prediction of the course of SARS-CoV-2-associated pneumonia of varying severity. The retrospective analysis included 370 case histories of patients who were treated at the COVID-19 medical care center in Kharkiv. The analysis included an assessment of the clinical course of pneumonia, the presence of olfactory and general otolaryngological symptoms, and the level of some blood laboratory parameters. Statistical analysis included discriminant analysis with calculation of normalized Euclidean distance to determine the diagnostic significance of clinical blood parameters. Data analysis showed that 52.2% of patients reported the occurrence of olfactory dysfunction, which was most often observed in patients with mild SARS-CoV-2-associated pneumonia (68.1%) and in patients without pneumonia (88.6%). Statistical analysis showed that adding clinical blood test parameters is advisable in patients with relatively mild forms of COVID-19, as it reduces the probability of diagnostic errors to 0.03. In patients with moderate and severe SARS-CoV-2-associated pneumonia, specific COVID-19 parameters already provide high diagnostic accuracy (error less than 0.02), and the inclusion of clinical blood test parameters only increases the accuracy of the discrimination model. Thus, olfactory disorders associated with acute rhinosinusitis in COVID-19 could be a prognostic marker of a milder course of pneumonia in hospitalized patients. At the same time, clinical blood test parameters have limited additional value in advanced diagnostics.
