Adherence to follow-up in first-episode psychosis: ethnicity factors and case manager perceptions

Objectif: Il existe des disparités dans l'accès aux soins de santé mentale et l'adhésion des patients de minorités visibles (VM). Les objectifs de cette étude étaient les suivants: 1) déterminer si les patients atteints de VM avec un premier épisode de psychose (FEP) sont plus à risque de non-adhérence au traitement que les patients blancs, et 2) susciter la perception des gestionnaires de cas des patients. Méthode: Les données concernant 168 patients référés à une clinique de psychose de premier épisode de 2008 à 2011 ont été recueillies par l'examen des dossiers. Pour 110 patients, un questionnaire rempli par le CM de chaque patient a recueilli des données quantitatives et qualitatives concernant les perceptions des CM sur la perspicacité des patients, leur coopération et leur adhésion aux rendez-vous et aux médicaments. L'observance du traitement différentiel chez les patients blancs et VM a été testée au moyen d'analyses du chi carré. Les estimations des CM d'adhérence ont été comparées à des données objectives par la kappa de Cohen. Les données qualitatives ont été analysées par analyse thématique. Résultats: Les patients noirs avaient un suivi plus faible par rapport aux autres patients (χ2 = 5,4, df = 1, p = 0,02). La concordance de l'adhérence rapportée par le CM et les données du diagramme étaient significatives pour le groupe VM seulement (kappa = 0,444, p = 0,002). Dans les perceptions de CM, il n'y avait pas de différence significative entre les groupes ethniques en ce qui concerne l'adhésion aux rendez-vous et les médicaments, la perspicacité ou la participation de la famille.Conclusions: Bien que le Canada soit souvent perçu comme tolérant à la diversité, nos données concernant le suivi chez les patients noirs indiquent des problèmes semblables à ceux signalés au Royaume-Uni et aux États-Unis. Les cliniciens peuvent avoir de faibles attentes pour les patients VM et donc remarquer de façon plus cohérente quand les patients VM adhèrent bien au traitement. Il s'agit de la première étude à examiner les différences ethniques dans l'adhésion au suivi FEP dans un contexte canadien.
