Osteogenesis imperfecta (OI), a heritable connective tissue disorder, often leads to long-bone deformation and bone fragility, significantly impacting daily activities. Current treatments involve bisphosphonates and surgical correction, including osteotomy and intramedullary rodding. However, post-surgical challenges such as delayed union or non-union at the osteotomy site remain, causing pain, bending of fixation rods, repeated fractures, and loss of mobility. Seeking alternative solutions, researchers have explored bone grafts and bone-marrow-derived mesenchymal stem cells (BM-MSCs) for bone regeneration, but these options have limitations. An emerging stem cell source, the autologous adipose tissue-derived stromal vascular fraction (aSVF), provides a promising alternative. aSVF comprises non-expanded cells from enzymatically digested adipose tissue, possessing anti-inflammatory, angiogenic, immunomodulatory, and regenerative properties akin to BM-MSCs. Despite extensive research on aSVF's regenerative abilities, its potential for healing in OI remains unexplored.In this study, we present the first-ever isolation and characterization of Stromal Vascular Fraction (SVF) from adipose tissue of OI patients. Our investigation included 42 adipose tissue samples from pediatric patients, comprising 38 diagnosed with OI (age range: 2–22 years; 19 males, 19 females) and 4 non-OI controls (age range: 4–14 years; 1 male, 3 females), undergoing corrective bone surgeries at Shriners Hospital for Children (Montreal, Canada). Adipose tissue weights averaged 6.70 g in the OI group and 7.48 g in controls, with comparable numbers of viable and nucleated cells isolated between the OI and control groups (OI aSVF viability: 85.6±3.8%, control group: 80.5±5.3%). Notably, OI subtype, gender, age, and harvest site did not influence the overall number of isolated viable cells. Detailed SVF cellular analysis revealed fewer endothelial progenitor cells in OI compared to controls, while other cell populations, including MSCs (CD45-/CD34+/CD73+/CD90+), exhibited similar distributions in both groups. Additionally, both control and OI MSCs demonstrated comparable differentiation capacities into osteogenic, adipogenic, and chondrogenic lineages.These findings strongly support the potential of SVF derived from OI patients' adipose tissue as a novel source of MSCs for autologous stem cell therapy in OI. This study lays the foundation for future research in OI treatment, emphasizing the regenerative capabilities of aSVF in bone healing and regeneration
