En matiere de lutte contre l'infection a V. I. H. , les therapies antiretrovirales prescrites a partir de 1996 ont permis sa chronicisation. Majoritairement contamines en raison de pratiques d'echange de seringues, ou dans une moindre mesure, de rapports sexuels non proteges, les detenus seropositifs ont pu beneficier de ces traitements dans le cadre des prises en charge C. I. S. I. H. Presentes comme multivulnerables, confrontes a differentes stigmatisations, parfois consideres comme des marginaux, il a semble utile d'etudier l'adhesion des detenus a ces medicaments ainsi que son impact sur leu evolution identitaire egalement modifiee par leur incarceration. Ce qui m'a amene a questionner la relation soignant-soigne et a interroger plus generalement les acteurs participant de l'univers carceral sur leurs buts. La these demontre que les detenus, toxicomanes ou non par ailleurs, sont adherents a leur traitement a 85 %, taux comparable a ceux obtenus dans la population generale, en raison des strategies de vie qu'ils adoptent, de leur metis, et du soutien que leur apporte le monde medical, le monde socio-educatif comme les associations. Ce qui leur permet de resister a la reification partielle et donc provisoire de leur identite, provoquee par la conception philosophique et politique du milieu carceral ainsi que par son fonctionnement, attenuee par ailleurs par le respect des droits dont ils y beneficient et par leurs relations avec certains surveillants. Dans ce contexte, ils parviennent a preserver leur identite et a acquerir des savoirs et des savoir-faires durables. Cependant le caractere totalitaire de la prison contribue a interroger son utilisation en tant qu'outil penal.
