Abstract
No estamos en la 3a revolucion en salud mental, desarrollo de intervenciones comunitarias, sino en una 4a, la prevencion y promocion de salud.. La sociedad globalizada y liberal tiene un estado menos controlador que tambien da menos apoyo. Aparecen ademas nuevas y numerosos desordenes emocionales que requieren atencion. La dificultad de extender la promocion de la salud en la prestacion de servicios deviene del modelo explicativo anclado en lo individual y en la enfermedad ademas de la dificultad para definir el concepto promocion de la salud. Lo que se ha hecho en la declaracion de Ottawa(1986). En la Region sudamericana se detecta un cambio de perfil epidemiologico y nuevos grupos de riesgo. Estabamos en los comienzos de la 3a revolucion y ya esta en el centro de la atencion la 4aque la Declaracion de Caracas se ha comprometido a impulsar. La colaboracion internacional del centro colaborador de Granada (Espana) a modo de consultoria avanza en el diseno de un plan de modernizacion y redefinicion organizativa de los servicios y redes asistenciales. Describimos sucintamente la situacion actual de la red y subprogramas en desarrollo en Chile.
