OP04.05: Is the crux of the heart normal in the conotruncal pathologies? About a series of 40,740 pregnancies

To examine if a new echographic abnormality of the crux of the heart: the linear insertion of the atrioventricular valves (LIAVV) can be found not only in the pathologies of the inlet but also in those of the outlet (conotruncal pathologies). A prospective study about the sonographic screening of the LIAVV. During 6 to 12 months, 42 physicians of the CFEF (French College for fetal Ultrasonography) practised 40740 examinations of 2nd and 3rd-trimester pregnancies, searching for a LIAVV (with or without defect). A previous training checked that all the physicians were able to visualize the crux of the heart and its offsetting, constant in normal. Out of 39835 interpretable examinations, 63 pathologies of the crux of the heart were found: − 35 LIAVV without defect were presented as an Oral communication, Vancouver, Sept 2005. − 80% (CI 95%: 0.76–0.84)in 35 were associated with a pathology. − 68% (CI 95%: 0.61–0.70)of 35 were trisomies 21. We present here new data about the − 28 LIAVV with defect also found in this study: − 7 had abnormal kayotypes: 5 trisomies 21, 1 trisomy 18. In all these 6 cases the defect belonged to the inlet. In the 7th case, a del 22q11, the defect was an outlet defect-21 had a normal karyotype: 2 Viscero atrial heterotaxias with a complete AVSD. 3 other showed an inlet defect (partial or complete AVSD). But the remaining 16 cases were Cono truncal pathologies: the defect was actually not in the inlet but in the outlet. 17 cases (1del 22 q11, 16 normal karyotype) out of 28 LIAVV with defect, 61% (CI 95%:0.55–0.66) were conotruncal pathologies. All these LIAVV, found on the 4 Chamber view, were associated with an outlet defect. That means that CT pathologies actually known as being prenatally diagnosed by exploring the outlet could be highly suspected on the fetal 4 chamber view when the crux of the heart is well examined.
