Effect of Community-Based Group Prenatal Physical Activity on Pre-eclampsia Rate

Disorders of pregnancy that are associated with inflammation are major contributors to poor pregnancy outcomes, prematurity and birth defects. Pre-eclampsia (PE) is one such disorder characterized by maternal inflammation and resulting in dysfunctions in immune, cardiovascular and metabolic systems. It is cured only by birth of the baby - usually preterm - and composes 15% of preterm births in the U.S.; rates are higher in the developing world. In non-pregnant people, inflammation is treated with and prevented by pharmaceuticals and lifestyle methods like physical activity (PA). Most medications are contraindicated in pregnancy, but PA during pregnancy has been found safe, healthy and beneficial for mom and baby. Evidence shows that PA can enhance pregnancy outcome, and decrease excessive inflammation and risk of PE. Previous epidemiological studies indicate that PA may be useful in preventing PE. However, these studies have quantified PA based on recall of postpartum women and have not controlled for differences in their interpretations of amount, type or intensity of PA. But, controlling these variables in the research by using a laboratory-based PA intervention inflicts great difficulty. Translating the laboratory-based PA into a community-based program is problematic. Previous epidemiological studies indicate that PA may be useful in preventing PE. However, these studies have quantified PA based on recall of postpartum women and have not controlled for differences in their interpretations of amount, type or intensity of PA. Translating the laboratory-based PA into a community-based program is problematic. PURPOSE: To determine the effect of standardized, community-based group PA on preventing PE. Method: A retrospective study was performed to determine the rates of PE among the 9,160 women who gave birth at Yale-New Haven Hospital (YNHH) during 2004-2005, and a 90 person subset of this group who performed prenatal PA in a standardized, community-based PA program for pregnant women. The PA program controls type and intensity of PA performed and has been offered in the community for 30 years. RESULTS: Total PE rate in 2004-2005 at YNHH was 7.8%. For the PA group, PE rate was 2.2%. Two women in the PA group were diagnosed with PE in the last month of pregnancy and delivered normal infants at term. No PE was seen in the PA group during the 2nd or early 3rd trimesters nor was there any prematurity in this group. CONCLUSION: Findings support the hypothesis that adequate physical activity in a standardized community-based group setting may be a non-pharmacological way to prevent PE.
