Inducing remission with nutritional therapy has better height gain in comparison to corticosteroid in children with Crohnʼs disease

Several studies have reported nutritional therapy to have an equivalent efficacy in achieving remission in Crohn's disease (CD) when compared with corticosteroids. However, the effect of these treatments on changing height and weight has not been reported in the pediatric population. In this retrospective chart review we aim to compare the efficacy of nutritional therapy with corticosteroid therapy in inducing clinical remission; as well as, the effect on nutritional status (% weight gain and δ height z-scores) within one year of diagnosis of CD. Pediatric Crohn's disease patients diagnosed from the last 7 years had clinical, laboratory, demographic and endoscopic data collected. Forty-four patients received corticosteroids (mean age 11.46 years) and 13 received nutritional therapy (mean age 12.93 years). Both nutritional therapy and corticosteroids had equivalent PCDAI score at the time of diagnosis. Both achieved a similar remission rate within the one year time frame. 84.6% of nutritional therapy patients compared to 86.4% of corticosteroid patients. The relapse rate was also similar with nutritional therapy at 38.5% and corticosteroids at 29.5%. Nutritional status was shown not to be significantly different in terms of weight gain; nutritional therapy patients showed a mean weight gain of 22.162%, while steroid patients had a mean of 23.315%. However, change in height z-scores were shown to be significantly different with nutritional therapy patients giving a mean change of 0.098, while corticosteroid patients had a mean change of -.127, (p<0.05). It appears that the efficacy of the two therapies is similar in terms of remission rates. Weight gain appears to be similar in both groups, while change in height z-scores was significantly greater in those patients receiving nutritional therapy, making nutritional therapy an ideal agent in inducing remission in children with Crohn's disease.
