Effect observation of dermabrasion in the shock phase of children extensive deep partial thickness burns

Objective To analyze the clinical effect of children with large area of deep Ⅱ degree burn shock stage eschar grinding and two methods in the treatment of eschar shaving,in order to find a more ideal method for treating children with large area deep burn wounds. Methods Sixty cases of large area deep burn hospitalized children were selected in department from 2008 February to 2012 February,including 32 male cases,28 female cases. The ages ranged from 1 to 4 years,averaged (3. 5±0. 5) years old,and total burn area was 15% -31% TBSA,the average was (20. 2±1. 9) % TBSA. According to different treatments,all the burn children were divided into escharectomy group and dermabrasion group,30 cases each group. After admission all the cases were deserved perfect preoperative examination and adequate fluidresuscitation. When staying stable vital signs,all the patients received tangential or eschar grinding operation under anesthesia within 24 hours after injury. The two groups were observed the infusion volume at shock stage,the incidence of postoperative complications and residual wound and the rate of skin grafting operation,the wound healing time,wound healing quality index. Results The first 24th hour infusion volume of the dermabrasion group (1865.2±220.2) mL less than that of the tangential group (2150.9±234. 5) mL(P0.05) . The incidence rate of postoperative complications (40%) and digestive disorders (13.33%) of the dermabrasion group were lower than that of the tangential group (93.33%,36.67%) (P0.05) . The incidence of residual wounds (13.33%) ,and skin grafting operation rate (6.67%) of the dermabrasion group were lower than that of the tangential group (36.67%, 26.67%) (P0. 05) . Wound healing time of the dermabrasion group (16.97±2.45) d was shorter than that of the tangential group (24. 22±1.12) d (P0.05) . Wound healing quality of the dermabrasion group Vancouver score (5.56±1.32) was below than that of the tangential group Vancouver score (9. 61±1.20) (P0.05) . Conclusions The treatment of eschar grinding at the shock stage has little effect on the vital signs of the children with large area of deep Ⅱ degree burn,and has the advantages of simple operation,less complications,side light injury,wound healing fast,light scar. It is a good and more practical method to treat large area of deep Ⅱ degree burn wounds early.
