Neurologii warszawskiego Uniwersytetu MedycznegoA -przygotowanie projektu badania, B -zbieranie danych, C -analiza statystyczna, D -interpretacja danych, E -przygotowanie maszynopisu, F -opracowanie piśmiennictwa, G -pozyskanie funduszy Opisano historię wprowadzenia do użytku metody badania ruchowych potencjałów wywołanych (MEP) przez stymulację mózgu człowieka zmiennym polem magnetycznym bez naruszania powierzchni ciała ludzkiego (TMS).Wymieniono najważniejsze historyczne i współczesne postaci odkrywców praw fizyki oraz twórców urządzeń umożliwiających zastosowanie wymienionej metody.Opisano ich osiągnięcia w zakresie dotyczącym tematu.W porządku chronologicznym przedstawiono odkrywców praw indukcji elektromagnetycznej, konstruktorów i pionierów badających jej wpływ na żywe tkanki człowieka.Motywy działania poszczególnych osób i ich naukowa spuścizna pozwoliły na zilustrowanie ciągłości rozwoju wiedzy w zakresie nauk mających szczególne znaczenie dla prezentowanego zagadnienia.Wśród danych z zakresu indukcji elektromagnetycznej oraz danych technicznych sprzętu stosowanego do stymulacji przedstawiono te, które mają szczególne znaczenie praktyczne przy planowaniu projektów badawczych i programów diagnostycznych.Przedstawiono zalety i ograniczenia metody.Odwołano się do własnej 35-letniej obserwacji postępu w zakresie wykorzystania metody oraz jej modyfikacji.Wyszczególniono liczne możliwości zastosowania metody w zależności od zagadnień klinicznych i poznawczych, ilustrując wybrane z nich przykładami z własnego doświadczenia.W tym celu odwołano się do wyników własnych badań.Niektóre z nich były prezentowane w latach ubiegłych.Wśród własnych doświadczeń w sposób syntetyczny przedstawiono wyniki badań w zakresie parametrów MEP u ludzi zdrowych, u chorych na stwardnienie zanikowe boczne, chorobę Parkinsona, niedowłady związane z reakcją konwersyjną i stwardnienie rozsiane.Własne badania przedstawiono dla zilustrowania różnych możliwości metody.Słowa kluczowe: historia, indukcja elektromagnetyczna, przezczaszkowa stymulacja mózgu.The history of development of transcranial magnetic simulation (TMS) as a method to elicit motor evoked potentials (MEP) is described.This history, in a chronological order, was shown by presenting most important persons who participated in discovering the relevant rules of physics, inventors, constructors, and finally physicians who adapted the mentioned method and technical devices for investigation in research and hospital laboratories.The factors which motivated those people and the knowledge they left behind served as an illustration of a continuous progress in the relevant biological sciences that culminated in the development of this special method for everyday use in clinical and research practice.Among the data on the phenomenon of electromagnetic induction and technical specifications of stimulators and coils, those which play a practical role in designing research projects and laboratory procedure patterns were specified.The advantages and limitations of the method were summarized.To illustrate the history of utilizing this approach during the last 35 years, and to illustrate some examples of applying the method in clinical neurophysiology, it was necessary to recall authors' experience.The phenomena and characteristics of MEPs evoked by TMS in healthy subjects and in patients suffering from amyotrophic lateral sclerosis, Parkinson's disease, hysterical paresis and multiple sclerosis were presented in brief.Some but not all of these results were presented or published before.
