107 Free Running Asthma Screening Test in Canadian Schoolchildren

This study was carried out to further develop the Free Running Asthma Screening Test (FRAST) as a practical efficient method to screen for asthma in Canadian schoolchildren in grades 4–12 (age 9–18). The FRAST was used to increase asthma awareness and identify new cases and to estimate the prevalence of asthma in Canadian children and adolescents in this age group. Agreement was obtained from school personnel and parents from 3 schools in BC and 3 schools in Ontario. The authors attended regular Physical Education (PE) classses to carry out the FRAST and an asthma teaching program. Instruction in the use of individual Peak Flow (PF) meter was done, the students practised the PF maneuver, then recorded 3 baseline PF measures. They then did a 7-minute outdoor run, followed by measuring and recording 3 PF's at each of 0, 5, 10 and 15 minutes post-run. During the time intervals between the PF measurements the students filled out an asthma screening questionnaire and recorded individual narrative comments about their symptoms with running. To score the results, the highest PF of each of the 3 PF's at each time was used and the percent change from baseline calculated. A drop of at least 15% from baseline PF to any of the post-run PF's indicated exercise-induced bronchospasm. Symptom scoring, history of previously diagnosed asthma and past or current use of asthma medications was elicited from the questionnaire and narratives. Of 1039 students present in PE classes on the FRAST days 1003 participated. The prevalence of a previous diagnosis of asthma, based on the questionnaire was 17.4% (n=101) in BC schools and 23.9% (n=101) in Ontario schools. The prevalence of asthma determined by FRAST was 13.8 (n=80) in BC and 24.6 (n=104) in Ontario. This yields an overall prevalence of asthma of 26.1% in the BC schools and 38.1% in the Ontario schools. Reported treatment rates of the total number of students with previously diagnosed or found to have asthma by the FRAST were only 10.6% in BC and 9.2% in Ontario. We conclude: mass asthma screening as part of the PE class is accurate and feasible in Canadian students in grades 4–12. Use of the FRAST as a provocation method, along with a questionnaire increases the number of cases found compared to just using passive questionnaires. There is a large treatment gap for this most common chronic disease of childhood.
