P0687 CHARACTERIZATION OF HEIGHT AND WEIGHT PATTERNS IN NEWLY DIAGNOSED CHILDREN WITH INFLAMMATORY BOWEL DISEASE (IBD)

Introduction: Growth failure complicates the course of up to 40% of pediatric patients with IBD. AIM: To characterize growth and factors affecting growth in newly diagnosed children with Crohn disease (CD) or ulcerative colitis (UC). Methods: Newly diagnosed patients from 18 US/Canadian centers were prospectively enrolled in an observational registry as of January 2002. Data on patients with 1-year follow-up were analyzed. Results: Among 33 Tanner 1–3 patients (mean age 10.4 yrs, 23 male, 27 CD, 6 UC), 9 (27%) (mean age 9.6 yrs, 8 CD, 1 UC) had growth delay (decrease in height-for-age % at diagnosis compared to pre-diagnosis height %). None of the patients who demonstrated growth delay had a BMI< 3%. The 1 patient with UC returned to pre-diagnosis height percentile by the 1st quarter, while all CD patients remained 1–3 channels below their pre-diagnosis height % at the 4th quarter. 7/24 (29%) (6 CD, 1 UC) without growth delay at diagnosis demonstrated a 1-channel decrease in height % by the 4th quarter. No difference was found in the PCDAI at diagnosis for those with (n=8) or without (n=19) growth delay, mean PCDAI =33.8 ± 6.3 vs. 36 ± 19.4 respectively, p=0.9. 6/33 (18%) were malnourished (BMI or weight/length <3%) at diagnosis, all of whom had CD. Malnutrition corrected by the 1st quarter in 4/6 patients. At 1 year, 1 patient remained malnourished; data were not available for the 2nd patient. PCDAI was significantly lower in patients with BMI>3% (mean=31.3 ± 14) than in malnourished patients (mean=49.6 ± 18), p=0.03. Albumin at diagnosis did not differ significantly between those with BMI>3% (mean=3.2) and those with BMI<3% (mean=2.6), p=0.12. Prednisone was used in 27/33 (82%) patients, including the 6 CD patients who were malnourished, 1 of whom remained malnourished throughout the 1-year follow-up period in spite of 2 courses of prednisone therapy. Conclusion: Almost 30% of IBD patients have growth impairment at diagnosis and catch-up growth is not usually seen 1 year later. An additional 29% of children with IBD with normal growth at diagnosis experience decreased growth velocity following diagnosis and start of therapy. Growth abnormalities continue to be a significant problem in this population suggesting new therapeutic strategies may be needed.
