Résumé

récupéré en direct d'OpenAlex

In 1986, by signing the Ottawa Charter, public health stakeholders formalized the health promotion approach and embraced the idea that it is necessary to act on various factors and levels of influence that make up an individual's environment in order to improve their health and the control they have over it. This approach to health is rapidly being conceptualized within settings, such as hospitals, schools, towns and, more recently, sports clubs. A number of health promotion interventions have been developed in sports clubs, and public health and political stakeholders are increasingly aware of the potential that this setting could have on health promotion. However, a number of limitations are emerging: it is difficult for health promotion interventions in sports clubs to be based on a holistic approach to health, and little is known about how to design, implement and evaluate these interventions.Keeping in mind these observations, the aim of this doctoral thesis was to develop and implement a health promotion intervention within sports clubs and to develop a protocol for evaluating it. To target the intervention strategies, an initial study was carried out to assess the sports participants expectations and perceptions of health promotion. The results showed that it was difficult to understand the possibilities for health promotion in sports clubs, and that the participants had little experience of health promotion interventions. The results have made it possible to develop ways of working for the development of the intervention. A second study, bringing together sports club staff, public health professionals and researchers, helped to co-construct the content and timing of the PROSCeSS intervention. Structured in 7 steps and comprising of a online training course, the aim of this intervention is to help clubs create an environment, an organisation, social relations, and activities that are conducive to health. Given the inherent complexity of adopting the socio-ecological approach within sports clubs, a third and a fourth study enabled to assess the effectiveness and implementation processes of the PROSCeSS intervention. Although the results of these studies were encouraging in terms of the acquisition of knowledge and skills for training, and showed that some sports clubs were able to promote health within their clubs after deploying the intervention, they also highlighted a number of barriers to implementation and learning, such as the lack of human and financial resources in the clubs and the lack of time on the part of those involved. Thus, with the aim of being deployed on a larger scale, the PROSCeSS intervention was reworked and a protocol, presented in a fifth study, was devised to be able to evaluate the implementation process and the effectiveness of the PROSCeSS intervention in the most appropriate way.In addition to having enabled the development of interventional content that could be reused by internal and external sports club actors wishing to promote health within this setting, this thesis questions the way in which interventional research in health promotion is constructed. Between assessing the expectations of target populations, participatory approaches, exploring implementation processes, and identifying appropriate evaluation methods, this work revisits some of the founding ideas of health promotion and questions the legitimacy of this project, particularly with regard to the sporting and societal system in which it is embedded.
