Aims & Objectives: Childhood critical illness impacts the entire family. Disruption to routines, relationships, shifts in caregivers and uncertainty can significantly impact siblings and other children connected to family. Addressing sibling needs can reduce patient and family anxiety, enhance family adaptation, and improve child and family outcomes. We will describe the potential impacts of a critical illness hospitalization on siblings and offer experience- informed approaches for providing developmentally aligned support to impacted children. Methods: A sampling of the foundational and current evidence related to sibling experience was accessed and complimented by the author’s combined 98 years of practice experience caring for critically ill children and their families. Recommendations are moderated by the authors and supported by lived experience. Results: The experience of the critical hospitalization of a sibling or close family member is unique to the sibling relationship, their age and stage of development and the context of the hospitalization. Disruptions in the family, changes in parental behaviors, exposure to unfamiliar situations, uncertainty and fear can be overwhelming to the well children in the family. Practical interventions that are principle-based, goal-directed and individualized are effective in surfacing and addressing the needs of siblings. Conclusions: Sibling wellbeing is essential to family adaptation in critical childhood illness. It is fundamental that critical care providers seek out and actively engage with families to plan how to best support siblings and younger family members. Practical interventions that enable consistent, transparent, and strength- based sibling care facilitate best outcomes for the entire family. Keywords: family centered care, Siblings
