Introduction: Osteoarthritis is by far the most common form of joint disease throughout the world. Knee osteoarthritis is an important contributor for global disability among the geriatric population. There are conflicting reports regarding radiological severity and clinical findings among patients. The aim of this study was to find the relation between radiographic features and severity of symptoms in knee osteoarthritis. Methods: This is a cross-sectional study conducted in Kathmandu Medical College Teaching Hospital, Sinamangal, Kathmandu. A total of 174 patients with osteoarthritis knee who presented from January 2019 to June 2020 were included in the study. The diagnosis of knee OA was based on American College of Rheumatology criteria (ACR) for knee OA. Demographic variables including age, sex, occupation and BMI were recorded. The severity of symptoms was evaluated using WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index) and radiographic features were evaluated using K-L grading. Correlation between WOMAC score and K-L grading was assessed for the knee OA. Results: Out of 174 patients (one hundred and thirty-three female and forty-one male) the mean age was 60.30±8.329 years (range: 50 to 94 years). Majority of our patients were housewives (37.9%) followed by farmers (32.2%). The average BMI of patients in our study was 27.40 (SD 2.41). Based on Kellgren Lawrence (K-L) grading, 13.2% patients had K-L grade I on X-ray of knee, 34.5% had K-L grade II, 35.1% had K-L grade III and 17.2% had K-L grade IV. The mean WOMAC score in our study was 43.99(Range; 24- 71). K-L grades were strongly correlated with WOMAC scores and sub-scores in our study with spearman’s correlation coefficient (rho) +0.784. Conclusion: Radiographic findings and severity of clinical symptoms were positively correlated. Knee pain, stiffness and limitation of function due to OA knee is strongly correlated with X-ray changes. Both clinical symptoms and radiographic findings should be considered when treating patients with knee OA.
