METABOLIC IMPACT OF SOCIAL TRANSITIONS IN CANADA: A PROSPECTIVE STUDY OF AGING WOMEN AND MEN IN THE CANADIAN LONGITUDINAL STUDY ON AGING (2011-2021)

Objective: We aimed to prospectively examine alterations in different social connections and change in waist circumference (WC) among aging Canadians, by gender Design and method: A population-based prospective study of middle-age and older adults in the Canadian Longitudinal Study on Aging (CLSA, 2011-2021) assessed change in marital status, living arrangements, and social participation (waves 1 to 2) and change in waist circumference (WC) at wave 3 (adjusted for wave 2). Stratified, multivariable mixed regression, and predicted means (95% CI), estimated effects by (cis-)gender adjusting for 9 individual and 3 provincial covariables. Sample sizes varied: 13,941 participants for marital and living arrangement transitions and 13,921 for changes in social participation. Results: Overall, mean WC declined over the 6-year follow-up time. We found that alterations in different social connections were associated with subsequent WC changed in a gendered pattern. Among women, only decreased social participation was significantly associated with subsequent change in WC among women (+0.47 cm [0.11, 0.83]) compared to women with stable social participation; although increased social participation also signaled less decline in WC (+0.32 cm [-0.04, 0.68]). Among men, only becoming divorced was significantly associated with subsequent changes in WC (+1.45 cm [0.14, 2.75]), compared to remaining partnered; notably, men becoming widowed had a similar magnitude but a non-significant inverse association (-1.41 [-2.85, 0.03]). Results for changes in living arrangement and subsequent change in WC were null but directions differed by gender. Predicted mean WC at 6-year follow-up were seen for uncertain marital transitions among women (87.95 cm [86.95, 88.94]) and for becoming divorced among men (100.54 cm [99.17,101.91]), placing both in the ‘high risk’ category for diabetes and coronary heart disease. Results were robust to alternative model specification and to further adjustment of all baseline social ties. Conclusions: This study revealed unique gender-specific patterns of change in social connections linked to WC change. Decreased social participation increased mean WC among women, whereas becoming divorced increased WC among men in Canada. Findings underscore the importance of disaggregated data and the unique role of specific changes in social connections for future risk of cardiovascular disease.
