#3609 Cognitive functions in multymorbidity patients with atrial fibrillation depending on the presence of concomitant chronic kidney disease stages 3A, 3B and 4

Abstract Background сognitive impairment (CI) is one of the significant public health problems. Multimorbidity patients with atrial fibrillation (AF) and chronic kidney disease (CKD) are at higher risk of developing CI, which can lead to deterioration in quality of life, difficulties in adherence to medication regimens and a higher incidence of adverse drug reactions, which determines the need to diagnose cognitive deficit in these multimorbid patients. Aims to assess the impact the stage of concomitant CKD on cognitive functions (CF) in patients with AF. Materials and methods we included 142 patients of both sexes ≥18 years old with AF in combination with CKD stages C3 and C4; 47 (33.1%) men and 95 (66.9%) women, aged 58 to 99 years (median age 84 [76; 90] years). Patients were divided into 3 groups: (i) with CKD C3a (n = 50), (ii) with CKD C3b (n = 50), (iii) with CKD C4 (n = 42). All patients underwent cognitive assessment including the Montreal Cognitive Assessment (MoCA), Mini-Mental State Examination (MMSE), Trial Making Test (parts A, B), Category Fluency, Word-List Recall and Stroop color-word conflict. Results patients with CKD C4 had a statistically significantly lower number of points on the MMSЕ, compared with patients with CKD C3a (27 [24;28] vs. 28 [26;29] points, respectively, P = 0.048). Among patients with CKD C4, there were statistically significantly fewer patients who scored ≥29 points on the MMSЕ, compared with patients with CKD C3a (8 (19%) vs. 19 (38%) patients, respectively, P = 0.047). Patients with CKD C4 reproduced a statistically significantly smaller number of words an hour after memorization than patients with CKD C3a (2 [0;3] vs. 3 [0;4] words, respectively, P = 0.017). To complete the TMT part A test, patients with CKD C4 required a statistically significantly longer time than patients with CKD C3a (89 [61;117] vs. 71 [58;91.5] seconds, respectively, P = 0.047). Conclusion the results obtained in this study indicate an adverse effect of concomitant CKD C4 on CF in patients with AF and a deterioration in CF with increasing severity of CKD.
