(121) Short Term Results and Safety Profile of Hyaluronic Acid Penile Girth Enhancement Using the PhalloFill Standardized Technique Procedure: Comparable Outcomes for Both Circumcised and Uncircumcised Men

Abstract Introduction Penile girth enhancement (PGE) procedures have increasingly been sought after for aesthetic and functional reasons for the care of penile dysmorphia disorder (PDD). Unlike other HA injection techniques with variable chain length, cross-linking, chemical modifications and external pH which that affect its structure and viscosity, the standardized PhalloFill technique and unique sleeve system offers security and reproducibility of outcomes with more even distribution and prevention of unwanted migration. Objective This study provides a retrospective review of an HA-based penile girth enhancement focusing on treatment outcomes and complication rates. Methods Data was retrospectively collected for 59 men who underwent HA-based dermal filler at BPH Canada Male Sexual Wellness Center, Montreal, Canada, from November 2023 to October 2024. Baseline demographics and clinical characteristics were recorded, including age, circumcision status, body mass index (BMI), and prior medical history such as erectile dysfunction and previous treatments. Preoperative measurements of mid, base, and apex penile girth were obtained, with patients receiving between 4 to 6 units of HA filler. Complications were tracked, and changes in penile measurements were assessed at 1-month post-procedure. Results The mean age of treated men was 44.2 years (SD 10.5), with a mean BMI of 26.6 (SD 7.4). Baseline penile dimensions were mean base-to-corona length 3.87” (range 2.24-5.4”), and mean girth at mid -phallus was 3.64” (range 2.5-4.5”). Men with circumcision and active smoking were observed in 49% and 37%, respectively. Initial treatments were 61%, 5% and of 34% of men were injected with 4,5 an6 units of HA filler. All men demonstrated significant mid-girth enhancement from baseline to 1 month, with a mean increase of 0.36 “ (SD 0.16, p < 0.01). There were no difference between girth enhancement between circumcised and non-circumcised mean (0.35”vs 0.36” respectively). Changes from baseline in apex and base girth were observed at 0.34 “ (SD 0.35, p = 0.56) and 0.31”(SD 0.28, p = 0.154), respectively. Overall complication rates were low, with mild skin erythema reported in 6 men (edge of sleeve treated with Polysporin) and 7.3% filler. All complications resolved with conservative management, and no long-term sequelae were observed. No Clavien-Dindo Grade >2 were observed. No injection-related infections, abscess, pain or hospitalization/ER visit was reported. No patient required dissolution hyaluronidase treatments. Conclusions Phallofill HA dermal filler with standardized treatment protocol using its unique penile compression sleeve for PGE is associated with significant, consistent girth gains across circumcised and uncircumcised men, with a low incidence of complications. Further studies are ongoing to assess subsequent treatments in girth gains and HA girth durability.
