Altered network oscillations and synaptic plasticity in the blood-brain\nbarrier-disrupted peri-infarct hippocampus are related to epileptiform\nactivity and impaired GABAergic inhibition

Aquired epilepsies in the elderly often result from brain lesions associated\nwith bloodbrain barrier (BBB)-disruption like stroke, trauma, tumors or brain\ninfections. Post-stroke epilepsy can thereby be related to cognitive decline\nand poor neurological outcome. Seizures and spreading depolarization arise as\ncharacteristic activity from ischemic lesions but may also contribute to\nlesion progression and reorganization of the adjacent neural network and are\ntherefore critical electrophysiological correlates of the pathophysiology. As\nan important structure for memory consolidation but also as a very sensitive\narea for epileptogenesis, the hippocampus is a crucial structure for\ninvestigating mechanisms underlying the pathophysiology of BBB-dysfunctional\ninduced changes in synaptic plasiticity and neural network activity. To\nelucidate mechanisms underlying the pathophysiology I investigated with a\nmulti-methodological approach different aspects of changes in rat hippocampus\nfollowing neocortical photothrombosis using magnetic resonance imaging,\nintracranial pressure and electrophysiological measurements as well as gene\nexpression analysis. Cortical photothrombosis was associated with early peri-\nischemic BBB dysfunction that included the underlying, ipsilateral hippocampus\nand increased intracranial pressure, both preceding the occurrence of\nvasogenic edema. Intrahippocampal field potential recordings revealed\nelectrographic seizures within the first week in two thirds of animals.\nPredominantly seizing animals displayed an increase in theta and reduction in\ngamma frequency bands indicating disturbed inhibitory activity. Synaptic\ninteractions and plasticity were studied in parasagittal hippocampal slices at\n24 hrs and 7 days post-stroke. Field potential recordings in CA1 and CA3\nuncovered multiple population spikes, epileptiform episodes and spreading\ndepolarizations at 24 hrs declining at day 7. However, input-output analysis\nrevealed that fEPSP-spike coupling was significantly enhanced at 7 days. In\naddition, CA1 feedback and feedforward inhibition were diminished over the\nfirst week. Slices generating epileptiform activity at 7 days revealed\nimpaired bidirectional long-term plasticity following high and low frequency\nstimulation protocols. Supporting these findings, microarray and PCR data\nconfirmed changes in expression of astrocyte-related genes and suggested\ndownregulation in expression of GABAA-receptor subunits. In conclusion, BBB\ndysfunction in the peri-infarct hippocampus is within the first week related\nto hyperexcitability, early disinhibition and abnormal synaptic plasticity.\nThus, the data reveal a strong connection between hippocampal neural networks\npresenting epileptiform activity and disturbed oscillatory activity as well as\nlong-term plasticity. These insights reveal possible new diagnostic monitoring\nopportunities in finding patients at risk for epileptogenesis and cognitive\nimpairment associated with peri-lesional BBB-dysfunction.
