This paper is an intervention project, the result of the Specialization Course in Primary Care at UFPR, funded by UNA-SUS.The Family Health Strategy (FHS) seeks to promote the quality of life of the Brazilian population and to intervene in factors that put health at risk, such as lack of physical activity, poor diet and tobacco use, with comprehensive, equitable and continuous attention. .We highlight the importance of strategies aimed at health care for the elderly, since the number of patients aged 60 years or older corresponds to 20% of the population in the area covered by the team.In addition, the agreement between the state and municipal primary care services and the elderly care networks is reinforced.Many elderly have health conditions or problems that contribute to the weakening and loss of independence to perform daily life activities.Added to these factors are situations of malnutrition, worsening of chronic diseases such as hypertension and diabetes, renal failure, lack of care and continuation with the offered cataract treatment and hearing evaluation.This increases the risk of the elderly losing autonomy and independence, and evolving into frailty, making them vulnerable to adverse outcomes.Thus, the general objective of the present intervention project was to develop health promotion actions for the elderly assisted at the Quebec Primary Health Care Unit identified with risk of weakening in the context of Primary Health Care.An action research was conducted, whose first phase was performed the individual care of the elderly applying the IVCF-20 instrument, according to municipal protocol.The elderly were stratified by clinical-functional vulnerability (fragile, at risk of weakening or robust) and the main factors of weakness of the elderly were identified.After identifying the weakening factors, a health promotion activity was elaborated, including discussion circles on depression and family insufficiency, on the elderly care network and available instruments to promote community life with a view to preventing depression, and a tea. the afternoon of fraternization, when the elderly group of Casa de Emmaus was introduced.At this last meeting, the participants were given a fridge message rack, made by the team.It was observed that there was an improvement in the quality of life of the elderly, according to reports and information from the users themselves during the following visits.
