SAMANDRAG\nBakgrunn: Diabetes er ein sjukdom som råker fleire og fleire, og opptil 1/10 av desse vil kunne utvikle komplikasjonen diabetisk fotsår. Diabetiske fotsår kan vere kompliserte å behandle, og det kan vere fare for amputasjon. I tillegg kan langvarig behandling vere kostbart både for samfunnet og pasienten sjølve. Dei fleste pasientane blir behandla i primærhelsetenesta. \n\nHensikt og problemstilling: Undersøke og syntetisere eksisterande forsking om korleis sjukepleiarar arbeidar kunnskapsbasert innan sårbehandling med diabetiske fotsår. Kva erfaring har sjukepleiarar i kommunen om oppfølging av diabetiske fotsår?\n\nMetode – design, utval og analyse: Databasesøk utført i Cinahl/Medline 21.12.21, søkt etter studiar på engelsk eller nordisk språk. Søket gav 241 unike treff, og 6 av desse møtte kriteria. Dei inkluderte studiane hadde eit utval på totalt 82 sjukepleiarar. Deltakarane arbeidde i kommunehelsetenesta i Noreg, Sverige, Danmark, og Canada. Kvalitetsvurdering med sjekkliste for kvalitativ forsking. Utført ei kvalitativ metasyntese, med tematisk syntese skildra av Thomas og Harden (2008). \n\nResultat: Metasyntesa avdekka at sjukepleiarane si erfaring var kjenneteikna av tre hovudtema; varierande kunnskapsnivå og forståing av kunnskapsbasert praksis; telemedisin – positivt hjelpemiddel for heilskapleg oppfølging av diabetiske fotsår; og kommunikasjon med andre profesjonar viktig. \n\nKonklusjon: Studia kan gje indikasjonar på at sjukepleiarar treng meir utdanning, vidareutdanning og kurs i oppfølging av diabetiske fotsår. Det synes og å vera behov for at leiinga legg til rette for kunnskapsbasert praksis. I tillegg vil det vera behov for meir forsking på sjukepleiar sine erfaringar i møte med diabetespasientar med fotsår, gjerne i regi av kommunen. \n\nNøkkelord: Diabetes – fotsår – sjukepleiar – kunnskapsbasert praksis – erfaring – metasyntese – kommunehelsetenesta\n \nABSTRACT\nBackground: Diabetes is a disease that affects more and more people, and up to 1/10 of these may develop the complication diabetic foot ulcer. Diabetic foot ulcers can be complicated to treat and there can be a risk of amputation. In addition, long-term treatment can be costly for both society and the patient. Most patients are treated in primary health care.\n\nPurpose: Investigate and synthesize existing research on how nurses work evidence-based with diabetic foot ulcers. What experience do nurses in the municipality have about following up diabetic foot ulcers?\n\nMethods – design, material, and analysis: Database search performed in Cinahl/Medline 21.12.21, searched for studies in English or Scandinavian languages. The search returned 241 unique hits, 6 of which met the criteria. The included studies have a total sample of 82 nurses. The participants worked in primary health care in Norway, Sweden, Denmark, and Canada. Quality assessment with checklist for qualitative research. A qualitative meta-synthesis was performed, with thematic synthesis described by Thomas and Harden (2008).\n\nResults: This Meta synthesis revealed that nurses' experience was characterized by three main themes; varying level of knowledge and understanding of evidence-based practice; telemedicine - a positive aid for holistic follow-up of diabetic foot ulcers; and communication with other professionals important.\n\nConclusions: The study may provide indications that nurses need more education, advanced training, and courses in the follow-up of diabetic foot ulcers. There also seems to be a need for management to facilitate evidence-based practice.
